Esophageal tuberculosis: role of endoscopic ultrasound in diagnosis

Abstract: Esophageal involvement by tuberculosis is rare and is commonly secondary to mediastinal lymph nodal involvement. Endoscopic ultrasound (EUS) is a good modality for evaluation of both esophageal wall and mediastinal lymph nodes. The objectives were to study the role of EUS in diagnosing esophageal tuberculosis, to differentiate primary from secondary form, and to assess the response. Retrospective analysis of data over 7 years (i.e. from 2003 to 2009) was used. The study was set in a tertiary care referral inst… Show more

“…This was reported from a center in India, a country where tuberculosis is endemic. Besides the presence of lymph nodes adjacent to the site of an esophageal pathological lesion (ulcer), a conglomerate mass of heterogeneous and predominantly hypoechoic lymph nodes with intervening hyperechoic foci or strands were characteristic of mediastinal tuberculosis …”

“…Table summarizes types of esophageal abnormalities caused by mycobacteria. Dysphagia and odynophagia are common symptoms often for weeks and months before the diagnosis is made . Biopsy for histopathology, smears, molecular tests, and cultures are essential to establish diagnosis.…”

“…Endoscopic biopsies usually disclose granulomar inflammation. Esophageal EUS should be performed as it allows to demonstrate the lymph node infiltration of the esophageal wall and to perform fine needle aspiration, thereby increasing the possibility of a positive diagnosis of tuberculosis [4]. PET-CT scan shows hypermetabolic mediastinal lymph nodes, but is not specific: sarcoidosis or metastatic lymph nodes are also hypermetabolic [3].…”
