Escitalopram vs duloxetine in acute treatment of major depressive disorder: meta-analysis and systematic review

Abstract: BackgroundPrevious evidence indicated that efficacy of escitalopram (Esc) and duloxetine (Dul) was comparable in the treatment of major depressive disorder (MDD). Since such studies had small sample sizes, this study purposefully applied a systematic review to determine the efficacy, acceptability, and tolerability those antidepressants in treatment of MDD.Participants and methodsThe following primary databases were searched in July 2017: Scopus, PubMed, CINAHL, and Cochrane Controlled Trials Register. Any ran… Show more

“…Major depressive disorder (MDD) is a recurrent, chronic mental illness and negatively impacts functioning and quality of life [ 1 ]. Although several antidepressants have displayed their efficacy in treatment in most of the MDD patients, including the first episode, [ 2 , 3 ] the delayed pharmacological antidepressant effects for the maximal or optimal improvement may require 6 to 12 weeks after administration, while some, including acutely suicidal patients, need a rapid response [ 4 ]. Additionally the delayed onset effect in MDD treatment is associated with several difficulties, including disability, reduced adherence to treatment, patient and family distress, economic consequences and increasing risk of suicidality [ 5 - 9 ].…”

“…A 2018 meta-analysis of three RCTs (N=1,120) compared the efficacy and tolerability of duloxetine with escitalopram in the treatment of MDD. 2 Patients were adults with MDD diagnosed using Diagnostic and Statistical Manual version IV or IV-TR criteria, and all had a score of 22 or higher on the MADRS. All patients were treated with an eight-week regimen of either 10 to 20 mg escitalopram or 60 to 120 mg duloxetine.…”
