Objective: Amifostine (AMI) has been used for the prevention of doxorubicin-induced cardiotoxicity in several experimental and a few clinical studies. The aim of this study was to investigate the effects of AMI on lipid peroxidation, protective enzymes, and mitoxantrone (MITO)-induced acute cardiotoxicity in the rat heart using biochemical tests and histopathological examinations. Materials and Methods: Thirty-six rats were divided into six groups (n=6 in each). Control rats were given intraperitoneal (i.p.) serum saline and AMI group rats were given 200 mg/kg AMI i.p. Rats received MITO-2.5 and 5 mg/kg i.p. in the MITO-2.5 and MITO-5 groups. AMI 200 mg/kg i.p. was administered 30 min. before the same doses of MITO in the MITO-2.5+AMI and MITO-5+AMI groups. Results: The levels of cardiac enzymes such as creatinine phosphokinase-myocardial band and cardiac troponin T did not change. Malondialdehyde (MDA) levels increased in MITO groups compared to controls. Catalase and glutathione (GSH) levels in the MITO and MITO+AMI groups were higher than in controls. Superoxide dismutase and glutathione peroxidase levels were not different between MITO groups and controls. There was no difference in MDA levels between MITO+AMI groups and controls. Calcium deposition was not detected. The scores of fibrosis, apoptosis, inflammation, and degeneration in MITO groups were higher than in controls. The scores of fibrosis, degeneration and inflammation in MITO+AMI groups were lower. Conclusion: MITO caused lipid peroxidation and myocardial damage, and the myocardium increased catalase and GSH levels to prevent this damage. AMI can protect against MITO-induced acute cardiotoxicity, decreasing myocardial damage and lipid peroxidation. (Turk J Hematol 2010; 27: 62-9)

Since arthritis induced by Mycobacterium products (adjuvant) in rats is considered to be immunologically driven, the objective of the present study was to determine if the immunosuppressor drug cyclosporin could affect hindpaw edema and joint hyperalgesia simultaneously. Female Holtzman rats (140-170 g) presented hyperalgesia and edema on the 8th and 12th day following adjuvant injection. Daily systemic (oral or intramuscular) administration of cyclosporin (0.5-5.0 mg kg -1 day -1 ) or dexamethasone (0.01-0.1 mg kg -1 day -1 ) for 15 days starting on day zero dose-dependently inhibited the hindpaw edema and hyperalgesia in arthritic rats. However, hyperalgesia but not edema could be detected two days after cyclosporin withdrawal. We concluded that a) the continuous presence of cyclosporin is essential to reduce the development of joint hyperalgesia and that b) different mechanisms underlie the appearance of hyperalgesia and edema in this model. The intracerebroventricular (icv) administration of 5-50-fold smaller doses of cyclosporin (1.5-150 µg/day) or dexamethasone (15 µg/day) also reduced the arthritic hindpaw edema and hyperalgesia. Peripheral blood from animals injected with effective systemic cyclosporin doses showed detectable levels of the drug, whereas peripheral blood from those injected with icv cyclosporin did not, as measured by specific RIA. Our results indicate that cyclosporin administered by the central route is as effective as by the systemic route to reduce joint hyperalgesia and hindpaw edema in arthritic rats. The antiarthritic effect induced by low doses of cyclosporin in the central nervous system (CNS) could be explored to avoid its often associated systemic side effects during chronic therapy. ObjectivesA longer postoperative care, needed for patients admitted to the hospital, is expensive and associated with increased morbidity and mortality, when compared with the outpatient setting. Outpatient therapy with continuous infusion of drugs with elastomeric pumps represents an effective method to address this problem. The aim of this work is to analyse the benefits in using elastomeric devices and to test their their behaviour towards drugs to changes during storage that could influence quality, safety and efficacy of the therapy. MethodsSeveral drugs belonging to different therapeutic classes, including anticancer, analgesic opioids, local anesthetics and Non-Steroidal Anti-Inflammatory Drugs (NSAIDs) have been studied using a combined HPLC/LC-MS approach. Each drug was loaded in three different brands of elastomeric devices and the samples were withdrawn over 7 days and submitted to HPLC/LC-MS analyses. Key-FindingsAll tested drugs showed high stability in each filled device, in fact only a low variability, less than 5 %, in term of percentage change in chromatographic areas, was observed. Moreover additional peaks, due to degradation of drug and/or to medical device-drug interaction, have not been detected both in HPLC and LC-MS analysis. ConclusionThanks to the implementation, within clinical protocols, of the use of these infusion systems, two important goals can be achieved: a) the keeping of the quality of care also out of hospitals and b) the reduction of tangible costs as well as intangible costs in health care.
