Erectile Impotence Treated With an Implantable, Inflatable Prosthesis

Abstract: Erectile impotence can now be treated with a device that mimics a natural erection. Between 1973 and 1977, we implanted an inflatable prosthesis in 245 men (235 with organic impotence and ten with psychogenic impotence). Of these, 234 are able to use the device to their satisfaction; no failures have occurred in the 152 cases treated in 1976 and 1977. The success of this treatment rests in part on the careful selection of patients by a team--a urologist, a sleep researcher, a psychologist, and a psychiatrist--… Show more

“…1 Improvements in design and technique have reduced the incidence of complications, but mechanical failure and infection remain troublesome if infrequent. We cannot be certain in our case whether the reservoir eroded into the bladder, or if it was inadvertently placed there.…”

“…4 This device, introduced in 1973, was the first to allow a man to have a prosthetic erection only when needed and to provide nearly natural flaccid and erect states. This implant would now be designated as a three-piece inflatable prosthesis: the three pieces being paired inflatable intracorporeal cylinders, a small scrotal pump and a large volume abdominal fluid reservoir.…”

“…The first in 1973 was the introduction of the inflatable penile prosthesis. 4 The second in 1983 was the introduction of intracavernous vasoactive drug injection. 5,6 The third in 1998 was the introduction of sildenafil citrate, the first significantly effective oral therapy for ED.…”

“…The three-piece inflatable penile prosthesis was introduced by F Brantley Scott et al in 1973. 2 Initially, this device had numerous mechanical breakdowns. Continued improvements in design, decreased chances of erosion, ability to conceal the device when deflated, and excellent rigidity when inflated, have provided patients with high satisfaction rates.…”
