Eradication success for non-tuberculous mycobacteria in children with cystic fibrosis

Abstract: Non-tuberculous mycobacteria (NTM) are an emerging pathogen worldwide in both cystic fibrosis (CF) and non-CF pulmonary disease (PD), with reports suggesting an increasing prevalence [1,2]. It is an opportunistic infection acquired from the environment [3], though conflicting evidence remains around person-to-person transmission [4,5]. Recent evidence suggests that Mycobacterium abscessus complex (MABSC) may be the most detrimental airway infection to lung function in CF [6], yet its treatment remains poorly e… Show more

“…Pulmonary mycobacteriosis is severe in individuals with a predisposition to infection and in individuals with comorbidities in the United States [ 22 ], South Korea [ 23 , 24 ], and China [ 25 ]. In particular, patients with cystic fibrosis are predisposed to colonization and NTM infection [ 26 , 27 ].…”

“…Eradication of M. abscessus can be achieved in 48 to 65% of children 95 96 and less often in adults. Treatment regimens for M. abscessus are not evidence-based and involve complex and often poorly tolerated drug combinations for prolonged periods (>12 months).…”

“…Treatment outcome data are very limited. In a retrospective review of pwCF from Colorado, the rate of eradication for MAC was 60% [38] and 67% among children with CF in London [39 ▪ ]. Treatment of pulmonary disease due to M. abscessus is complicated because of significant levels of in vitro resistance, the need for intravenous antibiotics, common adverse reactions, and generally poor outcomes.…”
