Eradication of Helicobacter pylori Using One‐week Triple Therapies Combining Omeprazole with Two Antimicrobials: The MACH I Study

Abstract: Omeprazole triple therapies given twice daily for 1 week produce high eradication rates, are well-tolerated, and are associated with high patient compliance. The two most effective therapies were those combining omeprazole, 20 mg, with either amoxicillin, 1 gm, plus clarithromycin, 500 mg, or metronidazole, 400 mg, plus clarithromycin, 250 mg, all given twice daily.

“…In our study, the OMC regimen was particularly disappointing, yielding an eradication rate of only 80.5% (95% CI: 65.1, 91.2%). In the MACH 1 study, 6 using the same treatment regimen, a higher eradication rate of 89.7% (95% CI: 84.2,95.2%) was achieved. The explanation for this inconsistency in eradication rates is not entirely clear.…”

“…Laine and co-workers 11 also achieved an eradication rate of 77% on intention-totreat analysis with a 1-week course, but using a higher dose of clarithromycin at 500 mg b.d. In the MACH 1 European multicentre study, 6 a low eradication rate of 79.5% (95% CI: 72.2, 86.8%) was reported on intention-to-treat analysis with a OAC regimen using a low dose of clarithromycin (250 mg b.d.). When a higher dose of clarithromycin (500 mg b.d.)…”

“…3 The amoxycillin + clarithromycin and metronidazole + clarithromycin regimens were chosen as controls for the proton pump inhibitor-containing triple therapies. The 14-day omeprazole + amoxycillin regimen was chosen to provide a proton pump inhibitor dual therapy control for the proton pump inhibitor triple therapies.…”

“…4 However, US data documenting the ef®cacy of 1-week proton pump inhibitor-based triple therapies are limited. We assessed the ef®cacy and tolerability of the best 1-week triple regimens, based on initial results from the MACH 1 Study, 3 in a large US multicentre study, and compared these triple therapies to dual antibiotic therapies without a proton pump inhibitor (to assess the incremental bene®t of a proton pump inhibitor) as SUMMARY Background: One-week proton pump inhibitor-based triple therapies are very popular in the US despite limited US data documenting ef®cacy. We assessed 1-week proton pump inhibitor triple therapies for Helicobacter pylori, and compared them to dual antibiotic therapies (to assess bene®t of omeprazole) and to omeprazole±amoxycillin (to assess bene®t of clarithromycin) in a large, randomized, US multicentre study.…”
