Epstein–Barr virus-associated peripheral T-cell lymphoma in adults with hydroa vacciniforme-like lesions

Abstract: We describe two Korean adult patients who had necrotizing papulovesicles mainly on their faces. Skin biopsy specimens showed perivascular and periadnexal infiltrate of atypical lymphoid cells with vasculitis in the dermis and subcutaneous tissue. In situ hybridization demonstrated a latent infection of Epstein-Barr virus in the majority of lymphoid cells in the dermis. These patients were diagnosed as having T-cell lymphoma. Interestingly, large granular lymphocytosis was found in the peripheral blood of Case … Show more

“…In this lesion, presence of intensive atypical lymphoid cells is observed histopathologically. Neoplastic cells are stained CD3 (+), CD20 (-) (12). In our patient, no such increase in atypical cells was observed and lymphocytic cells were positive both with CD3 and CD20.…”

“…Episodes of the disease appear precipitated by insect bites, to which the patients exhibit marked hypersensitivity. [46][47][48][49][50] The cutaneous infiltrates are EBV positive. A hemophagocytic syndrome may be seen, as expected from the EBV association.…”

“…3,10 HV-like lymphoma (HVLL) and systemic EBV-positive T-cell lymphoproliferative disease (LPD) of childhood were incorporated for the first time in the 2008 World Health Organization (WHO) classification of tumors of hematopoietic and lymphoid tissues in the subgroup of EBV-positive T-cell LPD of childhood. 11 HVLL is defined as an EBV-positive cutaneous T-cell lymphoma that occurs in children and less often in young adults 12 and may be associated with hypersensitivity to insect bites. Clinically, patients present with facial edema and recurrent vesiculopapular rash followed by ulceration and crusting.…”
