Epigenome-wide discovery and evaluation of leukocyte DNA methylation markers for the detection of colorectal cancer in a screening setting

Abstract: Background: Colorectal cancer (CRC) is the third most common cancer worldwide. If detected at an early stage, prognosis is good. Despite increasing evidence for the benefits of implemented screening programs, such as screening colonoscopy, compliance is rather low. Hence there is demand for non-invasive tests for the early detection of CRC with high acceptance in population-wide screening. The objective of this study was to identify and evaluate leukocyte DNA methylation patterns as a potential biomarker for e… Show more

“…This knowledge could be exploited further to understand the biology of the trait and ultimately to make predictions about phenotype by means of DNA methylation. There is a range of phenotypes explored with EWAS, like, for instance, autoimmune diseases [ 1 – 3 ], cancers [ 4 6 ] and psychiatric conditions [ 7 11 ]. However, not all of EWAS has resulted in the discovery of well-reproduced sets of genomic targets.…”

“…Many recent studies utilizing such techniques extend to analyses in cancer types other than breast, such as glioma ( 5 , 55 ), colorectal ( 56 ), and lung ( 57 , 58 ) cancers. In colorectal cancer, Heiss et al ( 56 ) extracted DNAm data from leukocytes and identified two CpG sites in the promoter region of KIAA1549L with significant differential methylation compared to controls. Then, using KIAA1549L promoter methylation in logistic regression models, they created prediction models for colon cancer with c -statistics accuracies of 0.69 in the screening setting and 0.73 in the clinical setting.…”

“…Therefore, early detection of CRC is important for reducing the incidence and mortality rates of the disease (33). Currently, colonoscopy is the gold-standard diagnostic test to identify colonic pathology (34,35). However, this approach is invasive, has low adherence, and is associated with potential risks and discomfort to the patient.…”
