Epididymal Tuberculosis: A Diagnostic Challenge, Sonographic Findings, and Differential Diagnosis

Abstract: A male patient was referred to radiology with scrotal pain and swelling. Prior to diagnosis, the patient had three sonograms for scrotal swelling, which progressed to a discharging scrotal sinus. At the second scan, a risk factor of past history of pulmonary tuberculosis (PTB) was noted. The first sonogram showed findings suggestive of left epididymitis, the second sonogram showed possible primary testicular tumor or epididymal tuberculosis (EPTB), the third sonogram showed epididymal pathology extending to in… Show more

“…Despite US being cheap, easy to perform, associated with minimal risk of radiation, and having remarkable reliability for assessing complications of epididymal TB, the examination has low specificity for the diagnosis of epididymal TB [ 17 ]. US is the gold standard for evaluating the scrotum, but not for epididymal TB [ 20 ]. Besides, renal US is necessary to evaluate the upper urinary tract for hydronephrosis frequently found in GUTB with prostate association [ 12 ].…”

“…18,19 Even though the US is a cheap, easy, minimal radiation, reproducible examination to help assess complications of epididymitis TB, such as stula or abscess formation, the features of epididymitis TB are not speci c. 17 US is the gold standard for evaluating the scrotum but is still not considered the gold standard diagnostic tool for tuberculous epididymitis. 20 Besides, renal US is necessary to evaluate the upper urinary tract for hydronephrosis, which often occurs in UGTB with prostate involvement. 12 On MRI, TB lesions may show a T2 hypointense picture, similar to TB lesions elsewhere.…”

“…CT or MRI have little value in diagnosing epididymal TB; however, they are important to diagnose tuberculosis in lung and kidney and provide support for the diagnosis of epididymal tuberculosis. 11,16 Ultrasonography of Tb epididymitis is characterized by enlarged, heterogeneous and hypoechoic lesion in the body and tail, associated with hypoechoic testicular lesions or discharging sinuse. 10 The combination of clinical, radiological, histopathological and microbiological finding all play an important role in the diagnosis of TBEO.…”
