ResumoOs cistos epidermoides constituem lesões congênitas, benignas e raras, que correspondem a cerca de 0,2% a 1,8% de todos os tumores intracranianos, sendo que em apenas 5% dos casos localizam-se no quarto ventrículo. Apesar de sua gênese na vida intrauterina, são geralmente diagnosticados entre a terceira e quinta década de vida em decorrência de seu padrão de crescimento muito lento. A imagem ponderada pela difusão da ressonância magnética é fundamental para a formulação do diagnóstico. O tratamento ideal consiste no esvaziamento do conteúdo cístico com ressecção completa da cápsula. Neste trabalho, é descrito um caso de uma paciente de 31 anos de idade com síndrome cerebelar, evoluindo com síndrome de hipertensão intracraniana. A sintomatologia era decorrente de hidrocefalia obstrutiva por cisto epidermoide localizado no interior do quarto ventrículo, confirmado pela anatomia patológica. Only 5% of cases located in the fourth ventricle. Despite its genesis in intrauterine life, are usually diagnosed between the third and fifth decade of life due to its very slow growth pattern. The imagem weighted by the diffusion of the magnetic resonance is essential to formulation of the diagnosis. The ideal treatment of choice is the empyting of the cystic contente with complete capsule ressection. In this work the case of a 31-year-old female with cerebelar syndrome evolving with intracranial hypertension. The symptomatology was due to obstructive hydrocephaly by an epidermoid cyst located inside the fourth ventricle, confirmed by the pathological anatomy.

Introdução: Cisto epidermoide (CE) é um tumor congênito benigno, originário do ectoderma. Ocorre na terceira e quinta década de vida. Corresponde aproximadamente a 0,2 a 1,8% dos tumores intracranianos. A localização no quarto ventrículo é rara. O exame de eleição é ressonância magnética (RM), mas a confirmação diagnóstica é através da anatomia patológica. O tratamento ideal é a exérese total. Relato do caso: Paciente sexo feminino, 27 anos de idade com história de cefaleia e ataxia da marcha há um ano. Resultado: RM de crânio mostrou presença de lesão expansiva no quarto ventrículo. Submetida a exérese cirúrgica total da lesão. Resultado da anatomia patológica foi CE. Conclusão: CE é uma lesão intracraniana benigna e raramente localiza-se no quarto ventrículo. Seu tratamento é cirúrgico e cursa com bom prognóstico.

Fourth ventricular epidermoid cyst – Case series, systematic review and analysis

Background: Epidermoid cysts are extra-axial, pearly white avascular lesions mostly found in the cerebellopontine region. They are slow-growing and mostly become symptomatic when they attain significant size. They do occur at other anatomical locations, but fourth ventricle is a rare location. Three representative cases with their outcomes are described here. Methods: The systematic review was done with adherence to predefined criteria. The studied variables were age, gender, duration of symptoms (DOS), clinical features, hydrocephalus (HCP), extent of resection, postoperative complications, outcome, follow–up, and recurrence. Statistical analysis was done to identify predictive factors for outcome. Results: Final analysis included 58 studies containing 131 patients. The most common clinical feature was cerebellar dysfunction (93%). The most common cranial nerve involved was the abducens nerve ( n = 37, 28.46%). Preoperative HCP was present in nearly a third (35%) of patients. The outcomes were not different with age ( P = 0.23), gender ( P = 0.74), DOS ( P = 0.09), and HCP ( P = 0.50). Improved outcomes were associated with total resections ( P = 0.001), absence of preoperative cranial nerve dysfunctions ( P = 0.004), and presentation with features of raised intracranial pressure ( P = 0.005). Longer DOS (mean 76.74 months) was associated with significantly increased cranial nerve nuclei involvement ( P = 0.03). Aseptic meningitis was reported in 14.5% of cases. Recurrences were infrequently reported ( n = 9). Conclusions: Although the fourth ventricular epidermoid lesions are difficult to detect in an innocuous stage, when found, they should be extirpated early and totally, as a longer DOS leads to cranial nerve dysfunctions and suboptimal outcomes.
