Epidemiology, treatment, disposition and outcome of patients with acute exacerbation of COPD presenting to emergency departments in Australia and South East Asia: An AANZDEM study

Abstract: Patients treated in ED for AECOPD commonly arrive by ambulance, have a high admission rate and significant in-hospital mortality. Compliance with evidence-based treatments in ED is suboptimal affording an opportunity to improve care and potentially outcomes.

“…While first ED ABG was an objective assessment of oxygenation and reflected the early oxygen delivery processes in EMS and ED that were suggested to contribute most to mortality from hyperoxemia, it posed a limitation. ABG was performed in ED in a quarter to two thirds of all AECOPD patients in a multi‐centre study by Kelly et al The inclusion of AECOPD patients without ABG might change our results. Our patients were generally lucid without excessive carbon dioxide retention.…”

“…COPD is a common respiratory disease encountered in clinical practice. AECOPD worsens lung ventilation function and causes progressive damage, 18 resulting in heavy economic burden to patients and families. 19 Western medical treatments for AECOPD are mainly based on symptomatic routine treatment.…”

“…A multicentre study examined COPD exacerbations presenting to 46 emergency departments across Asia, New Zealand and Australia . Over 90% of patients had a prior diagnosis of COPD and the median duration of symptoms prior to presentation was 3 days, highlighting the opportunity for better use of community‐based interventions that can reduce emergency department presentations.…”

“…Over 90% of patients had a prior diagnosis of COPD and the median duration of symptoms prior to presentation was 3 days, highlighting the opportunity for better use of community‐based interventions that can reduce emergency department presentations. Other issues identified were underuse of blood gas analysis and underuse of non‐invasive ventilation (NIV) in those with respiratory acidosis . An editorial called for greater use of evidence‐based pharmacological and non‐pharmacological therapies that reduce COPD exacerbations …”
