Epidemiology of sepsis and septic shock in intensive care units between sepsis-2 and sepsis-3 populations: sepsis prognostication in intensive care unit and emergency room (SPICE-ICU)

Abstract: Background Diagnosing sepsis remains difficult because it is not a single disease but a syndrome with various pathogen- and host factor-associated symptoms. Sepsis-3 was established to improve risk stratification among patients with infection based on organ failures, but it has been still controversial compared with previous definitions. Therefore, we aimed to describe characteristics of patients who met sepsis-2 (severe sepsis) and sepsis-3 definitions. Methods This was a multicenter, prospective cohort stud… Show more

“…Sepsis is a clinical syndrome with physiologic, biologic, and biochemical abnormalities caused by a dysregulated host response to infection, which leads to life-threatening organ dysfunction 13 . About 30% of patients admitted to general intensive care units (ICUs) and about 80% of ICU patients with infection were found to meet the criteria of sepsis 14 , 15 , although the results might differ when different criteria were used. The management of sepsis is still challenging, with a mortality rate ranging from 10 to 50% 16 18 .…”

“…Applying different sepsis definitions leads to variable assessments of sepsis epidemiology. For example, while overlap is quite high [51], Sepsis-2 definitions [52,53] identify some patients not identified when Sepsis-3 [54] criteria are applied, and vice-versa. Similarly, different data sources affect estimates of the incidence and mortality [55].…”

“…Sepsis-2 defines septic shock as "a state of acute circulatory failure characterized by persistent arterial hypotension unexplained by other causes". The key criterion is hypotension "unexplained by other causes" [53]. Importantly, these "definitions" are really clinical criteria by which septic shock in patients might be recognized.…”

“…Data were collected by the SPICU ICU investigators as part of the routine clinical workup. Data collection methods have been described in a previous study [23]; the investigators entered data into an online standardized template. Patient information included demographic characteristics, admission source, comorbidities, frailty, sites of infection, sepsis-related severity scores including the Sequential Organ Failure Assessment (SOFA) score and the Acute Physiology and Chronic Health Evaluation II score, and laboratory data.…”
