Background: The Surgical Implant Generation Network (SIGN) Fin nail is a relatively newer design of intramedullary nails that eliminates the requirement for distal locking. While various factors influencing treatment outcomes with different nail designs have been extensively studied, limited research has focused on the specific implications of using the SIGN Fin nail. Objective: This study aimed to assess the relationship between Fin nail canal fitting and treatment outcomes in femur fractures treated with the SIGN Fin nail at Muhimbili Orthopaedics Institute. Methods: A cross-sectional study was conducted at a tertiary hospital in Tanzania. Patients who underwent femoral fracture fixation with SIGN Fin nails between January 2016 and December 2021 were recruited from the SIGN surgical database. Radiographic measurements were performed using radiant software, fracture union at one year was assessed using the mRUST score, and weight-bearing status at 6 weeks was obtained from patient records. Results: The mean canal diameter was 11.30±1.75 mm, and 82.2% had a canal fill of 80% or more. The reoperation rate was 5.9% (7/118). Although not statistically significant, patients with canal fill less than 80% had a higher rate of reoperation (p = 0.074). The union rate was 84.1% in this study, with a mean mRUST score of 9.4, and those patients with a canal fill of less than 80% were more likely to have nonunion at 1 year (p = 0.028). Pain on weight bearing at 6 weeks was reported by 42.9% (24/118) of participants, and this was significantly associated with canal fitting (p < 0.001). Conclusion: The study found the degree of canal fitting of the Fin nail to be an important factor affecting the outcomes of femoral fractures treated with SIGN Fin nails. A higher degree of canal fill was associated with better pain outcomes and lower nonunion rates.

Résumé Introduction Le traitement traditionnel des traumatismes des membres par des tradipraticiens est omniprésent dans les sociétés africaines au sud du Sahara. Ces pratiques sont source de nombreuses complications. Cette étude se propose de préciser le profil de ces complications et d’identifier les facteurs favorisant le recours à ces tradipraticiens. Matériel et méthodes Étude descriptive et analytique sur 12 mois, du 1er février 2018 au 31 janvier 2019, concernant tous les patients ayant consulté dans le service de chirurgie du Centre hospitalier universitaire Le Bon Samaritain de N’Djamena (CHUBS) pour une complication de fracture ou de luxation au niveau d’un membre qui avait été prise en charge par un tradipraticien. Le recueil des données a été fait à l’aide d’un questionnaire préétabli avec un recul moyen de 14 mois. Résultats Sur 144 patients atteints de traumatismes des membres, 47 souffraient au moins d’une complication de fracture ou de luxation suite à un traitement traditionnel, soit 33 % des cas. Parmi eux, 32 ont été retenus pour l’étude. L’âge moyen était de 23 ± 3 ans (extrêmes de 10-61) et le sex-ratio de 2,6 en faveur des hommes. La provenance était majoritairement rurale. Les élèves/étudiants et les cultivateurs/éleveurs étaient les plus représentés. Les accidents de la circulation routière étaient les plus rencontrés (n = 20). L’influence de l’entourage (n = 14) a représenté le premier facteur conduisant au choix d’un traitement traditionnel. L’attelle en bois n’immobilisant pas les articulations proximales de la fracture, associée au bandage ischémiant à l’origine des gangrènes, a constitué le premier mode de contention (n = 15). Le délai moyen entre le traumatisme et le début du traitement traditionnel était de 8,5 heures. Le délai moyen entre le traitement traditionnel et la survenue des complications était de 106 jours (extrêmes de 1-302). L’œdème du membre, le cal vicieux, la gangrène et la pseudarthrose ont représenté les complications les plus fréquentes. La lésion initiale a été une fracture fermée dans la plupart des cas (n = 22) avec une prédominance des membres inférieurs (n = 22). La prise en charge hospitalière a été chirurgicale (n = 19) ou orthopédique (n = 13). L’évolution thérapeutique a été bonne, assez bonne ou mauvaise, respectivement dans 24, 2 et 6 cas. Conclusion Les complications des traitements traditionnels des lésions traumatiques des membres sont graves. Une formation des tradipraticiens sur la connaissance des notions élémentaires d’immobilisation et la reconnaissance des signes de gravité, leur collaboration avec les structures de santé ainsi qu’une gratuité des soins dans les services hospitaliers permettraient de réduire ce phénomène. Les médias et réseaux sociaux devraient permettre de toucher un large public.

Results of surgical treatment of distal humerus fractures in adults in Cotonou

Background: Fractures of the distal humerus represent 1.3% of fractures in Cotonou. Their treatment is often surgical. The aim of this work was to assess the results of their osteosynthesis in a disadvantaged environment.Methods: This retrospective study from 2001 to 2015 included 38 operated patients for distal humerus fracture with a minimum follow-up of 12 months. The average age was 33.6 years; the predominance was male (27 cases). The major etiology was traffic accidents (31 cases), According to the AO classification, the fractures were type A (10 cases), type B (02 cases) and type C (26 cases). The average time of surgery was 21 days. The Lecestre plate was the most used (29 cases). An iliac cortico-cancellous graft was used in 03 cases. The mean hospital stay was 20 days.Results: The mean follow-up was 37 months. Functionally, the mean Mayo elbow performance score (MEPS) score was 66.3 points. The results were excellent (08 cases), good (13 cases), average (10 cases) and bad (7 cases). The patients were satisfied or very satisfied in 29 cases. Radiographically, the reduction was anatomical in 34 cases. Anatomical reductions were obtained with Lecestre plates. Consolidation was achieved in 36 cases within an average of 9 weeks. As complications we noted: radial nerve paresis (03 cases); secondary displacement (02 cases); superficial infection of the operating site (02 cases), wire migration or screw retraction (05 cases), non-union (03 cases), malunion in varus (04 cases) and osteitis of the proximal ulna (01 case).Conclusions: Fractures of the distal humerus are infrequent. The anatomical and functional results despite little technical support provided and the long delay before surgery are quite good.
