Introduction and Aim: In recent years, there has been an increase in the number of patients with renal pathology in most of the cases without severe symptoms and with significant changes in kidney function, when there is a need for renal replacement therapy (RRT). The objective is to analyze the need and availability of renal replacement therapy in patients with chronic kidney disease. Materials and Methods: Statistical data from the State Statistical Committee of the Republic of Azerbaijan, and Republican Clinical Urological Hospital named after Academician M.J. Javad-Zadeh were used. The dialysis centers and the average load per center in Azerbaijan for the period from 2014-2019 are analyzed. Results: The provision of dialysis machines in Azerbaijan is 7.1%, in economic regions it is on average 7.7%. The highest provision is in the Nakhichevan (10.9%), Ganja-Gazakh (10.1%), and Apsheron (8.9%) economic regions. In the Republican Clinical Urological Hospital named after Academician M. Javad-zade, the need for hemodialysis centers exceeds by 31.6%. Conclusion: With an increase in the number of dialysis machines, there is an increase in the average load per dialysis center. In the economic regions of the republic, the actual number of hemodialysis machines the need meets the standard (>50%).

Chronic kidney disease (CKD) is characterized by increasing prevalence, catalyzing properties in relation to cardiovascular and general mortality, and, in most cases, is asymptomatic, which means late diagnostic verifiability. The global average prevalence of CKD is 13.4%, and CKD C3-5 is 10.6%. The main causes of CKD C5 are diabetes mellitus (DM, 46.9%), hypertension (28.8%) and to a lesser extent, glomerulonephritis (7.1%) and polycystic diseases (2.8%), while other causes account for a total of 14.4%. Despite the simple diagnosis of CKD, one of the key problems of modern therapeutic and pediatric clinics is its low detection rate at the early stages, which, according to some data, reaches 96.6%. This review provides data on the criteria for the diagnosis of CKD, as well as more detailed consideration of the course of CKD in patients with DM, hypertension, and heart failure. Attention is paid to the medicinal origin of CKD, as well as to the development of anxiety and depressive disorders in CKD. General issues of treatment of patients with CKD are considered in detail. Lifestyle changes are an important part of the fight against the development and progression of CKD. Currently, Smoking, alcohol, and physical inactivity have been shown to have a harmful effect on the risk of developing and progressing CKD. Diet plays a certain preventive role. The main drugs with nephroprotective properties are angiotensin-converting enzyme inhibitors and angiotensin II receptor antagonists. Both classes of drugs are effective in proteinuric forms of nephropathies and in combination/association of CKD with diabetes or hypertension. The review also provides data on the nephroprotective properties of mineralocorticoid receptor antagonists, endothelin receptor antagonists, and sodium-glucose co-transporter-2 inhibitors. Given the high importance of identifying and effectively treating patients with CKD, it is necessary to focus on early detection of CKD, especially in high-risk groups. It is necessary to raise public awareness by creating and implementing programs for primary prevention of CKD, as well as awareness of patients, motivating them to follow the doctor's recommendations for a long time, including as part of the implementation of a non-drug strategy to combat CKD. It is important to use the full range of methods of drug therapy for CKD, including measures of universal nephroprotection. It should be remembered that the cost of late diagnosis of CKD is a reduction in life expectancy, primarily due to high rates of cardiovascular mortality, disability, and high-cost medication and kidney replacement therapy.

Background. Chronic kidney disease (CKD) is one of the most serious and pressing health problems worldwide today. The end stage of CKD leads to disability and dramatically reduces the quality of life, which is caused by the need for continuous renal replacement therapy. The causes of CKD are various and are often caused by urological pathology. Materials and methods. Five hundred eighty seven case histories of patients with CKD on renal replacement therapy in the Krasnoyarsk Territory for 2022 were analyzed. Most of them were patients with chronic glomerulonephritis (38%) and urological diseases (29%). Other reasons that led to hemodialysis are diabetic nephropathy (12%), hypertensive nephropathy (8%), mixed nephropathy (5%). Systemic diseases with kidney damage, oncological diseases, rejection of a kidney transplant, genetic diseases 2% each. Results. Оne hundred and seventy patients with urological diseases are on program hemodialysis. The mean age of this group of patients was 56.2 years. The incidence of CKD varies slightly between men (48%) and women (52%). The structure of the incidence of urological pathology is as follows: all patients somehow had signs of chronic pyelonephritis. Of 170 people, only 15% had primary chronic pyelonephritis as the main cause of end-stage CKD. The remaining 81% of patients showed signs of secondary pyelonephritis. Patients with polycystic kidney disease accounted for 44%, patients with urolithiasis 13%, patients with anomalies in the development of the kidneys and urinary tract 7%, vesicoureteral reflux, with prostate adenoma 5% patients with signs of a neurogenic bladder 3% and 3% patients with ureteral structure. Patients with oncological pathology accounted for 4%. Conclusion. Over the past decade, the structure of diseases of patients on hemodialysis has changed. Due to the more accessible diagnostic service in the region, the proportion of urological pathologies in patients is increasing. At the same time, the timely use of reconstructive, minimally invasive, nephron-sparing operations can reduce the risks of progression of CKD. However, the number of end-stage patients is steadily increasing, requiring continued development of the hemodialysis service and other forms of renal replacement therapy.
