(1) Background: Pelvic fractures (PFs) are related to osteoporosis, and represent a serious individual and socioeconomic burden. (2) Methods: We examined age- and sex-standardised incidence rates (SIRs) of PF, along with rates of all-cause overall and one-year mortality among patients with PF. We compared the mortality rates between PF patients and a matched fracture-free cohort. Patients ≥ 50 years old in Austria hospitalised with PF in 2010–2018, along with their dates of death, were recorded. (3) Results: We identified 54,975 patients with PF, of whom 70.9% were women. Between 2010 and 2018 the SIR of PF increased in men by 10.0%—from 125.3 (95% Confidence Interval 118.9–132.0) to 137.8 (95% CI 131.8–144.0) per 100,000—and in women by 2.7%—from 218.7 (95% CI 212.0–225.6) to 224.7 (95% CI 218.3–231.3) per 100,000. The one-year post-PF mortality rate was higher in men than in women (13.0% and 11.1%, respectively; p < 0.001). Pelvic fracture patients aged ≥ 65 had an elevated mortality risk (Hazard Ratio 1.75, 95% CI 1.71–1.79, p < 0.001) compared to controls. (4) Conclusions: There is a clear increase in the incidence of PF in the elderly population, with a greater increase in men over time. Pelvic fracture itself contributes to increased mortality in individuals aged 65 and above.

Purpose Trauma causes over 4 million annual deaths globally and accounts for over 10 % of the global burden of disease. Trauma patients often sustain multiple injuries in multiple organ systems. This study aims to investigate the incidence of orthopaedic injuries in adult trauma patients.MethodsThe study was a register-based study using data from the national Swedish trauma register – SweTrau. The inclusion criteria for the register are all patients with a trauma that activated the trauma team at the admitting hospital, or patients admitted without trauma team activation but with NISS (New Injury Severity Score) >15. By categorizing Abbreviated Injury Scale (AIS) codes into different orthopaedic injury types, we provided a detailed description of what types of orthopaedic injuries that occurred in trauma patients.ResultsA total of 37266 patients were included in the study. A total of 15246 (40%) patients had sustained any orthopaedic injury, and 7733 (51%) patients had more than one orthopaedic injury. Spine injuries were the most common type of injury affecting 19% (n= 7083) of all trauma patients, followed by injuries of the upper extremities (n=6273, 17%) and lower extremities (n=5943, 16%). Fractures were the domination type of orthopaedic injury (n=30755, 87%). Traffic injury was the most common injury mechanism with almost half (n= 18112, 49%) of the patients being admitted after some sort of traffic related incidentConclusionOrthopaedic injuries are common in trauma and occur in 40% of trauma patients. Slightly more than half of the patients with orthopaedic injury had ≥2 orthopaedic injuries.

Previous literature has provided conflicting results regarding the associations between early surgery and postoperative outcomes in elderly patients with distal femur fractures. Using data from the Japanese Diagnosis Procedure Combination inpatient database from April 2014 to March 2019, we identified elderly patients who underwent surgery for distal femur fracture within two days of hospital admission (early surgery group) or at three or more days after hospital admission (delayed surgery group). Of 9678 eligible patients, 1384 (14.3%) were assigned to the early surgery group. One-to-one propensity score matched analyses showed no significant difference in 30-day mortality between the early and delayed groups (0.5% versus 0.5%; risk difference, 0.0%; 95% confidence interval, −0.7% to 0.7%). Patients in the early surgery group had significantly lower proportions of the composite outcome (death or postoperative complications), shorter hospital stays, and lower total hospitalization costs than patients in the delayed surgery group. Our results showed that early surgery within two days of hospital admission for geriatric distal femur fracture was not associated with a reduction in 30-day mortality but was associated with reductions in postoperative complications and total hospitalization costs.
