Epidemiological trends of HIV‐1 infection in blood donors from Catalonia, Spain (2005‐2014)

Abstract: Non-B subtypes, circulating recombinant forms, and transmitted drug-resistance mutation sequences circulate among HIV-1-positive blood donors in Catalonia. Continuous local epidemiological surveillance is required to implement optimal prevention strategies for controlling transfusion-transmitted HIV and to improve health policies regarding HIV infection.

“…The rate of HIV-1 DR rate was 12.1% (43/356) is similar to that reported in the United States (13.0%) [30], Brazil (16.3%) [31] and some European countries such as Spain (14.0%) [32], but higher than Asia (4.6%) [33], The national (4.5%) [34], Chengdu (1.3%) [12], Guangxi (3.2%) [35] and five Blood center in 2007-2010 (4.4%) (Liuzhou, Kunming, Urumqi, Mianyang, Luoyang) [36], the difference may be caused by the continuous application of antiretroviral therapy (ART), so increasing the resistance of HIV-1. In this study, the HIV drug resistance rate in Chongqing, Urumqi and Mianyang was roughly the same as in previous studies.…”

“…In Spain, donors are deferred for 12 months for sex with more than one concurrent partner, or sex with an occasional partner. A recent study from Catalonia, Spain, evaluating HIV infection rates in donors from 2005 to 2014, found an overall prevalence of HIV of 7·7 per 100 000 donations; rates in repeat donors were only slightly lower than in first‐time donors . Ten of the 214 infected donors (4·7%) had NAT only positive results, indicating recent infection.…”

“…Several countries, including Spain, Portugal and Italy, have implemented policies based on sexual behaviours considered to be at higher risk, regardless of whether the partner is same sex or opposite sex; these may be referred to as ‘gender‐neutral’ or ‘risk‐based’ deferral policies . Higher risk sexual activities may include sex with a new partner or with multiple partners, particularly if the partner's risk behaviour is unknown.…”

“…However, the positively selected positions detected, and particularly nef H89F, might be associated with the already reported high plasma viral loads in the infected patients, that could have facilitated transmission. At this point, it is important to note that variants from this cluster have already been detected in seven Spanish regions, the last cases described in Catalonia (Bes et al, 2017). Also, unpublished results have recently shown the rapid expansion of a new F1 subcluster, closely related with the Spanish variant, presenting 188 reported cases between MSM (Vinken et al, 2017).…”
