Carbapenem-resistant Pseudomonas aeruginosa (CRPA) has been a major threat to human health due to its increased morbidity and mortality in clinical settings. Carbapenemase genes are less frequently found in CRPA compared with carbapenem-resistant Enterobacterales , of which carbapenemase producers are common. In this study, we identified 11 bla KPC-2 -harbouring P. aeruginosa isolates from 139 carbapenemase-insensitive P . aeruginosa isolates collected between 2010 and 2021 in a tertiary hospital in China. Nine isolates belonged to ST697, while the other two were ST463. The antibiotic susceptibility testing showed that all the isolates were multidrug resistant, including resistance to imipenem, meropenem, ceftazidime, and tigecycline. Patients with Klebsiella pneumoniae carbapenemase-2 (KPC-2)-producing P. aeruginosa infections were mostly associated with complicated diseases and prolonged hospital stay, with 30% deterioration. The whole-genome sequencing analysis showed that these isolates carried multiple antibiotic resistance genes and virulence genes, and the KPC-2 genetic elements were highly related in ST697 isolates. The complete sequencing of ST697 isolate SE5416 showed that the harbouring of bla KPC-2 resulted from complex transposition and homologous recombination of an Inc pRBL16 plasmid and other mobile elements. The Galleria mellonella infection model experiment showed that these KPC-2-producing P . aeruginosa –infected larvae had low survival rates and high virulence. The present study revealed the shifting of CRPA from ST697 to ST463 in East China; ST463 had higher drug resistance, posing greater challenges for clinical management.

At presently availability of resistance pathogen to numerous drugs is a common feature of hospital-acquired Pseudomonas aeruginosa strains. So, the present study collect the 80 blood sample from blood cancer patient in Revathi Medical Hospital, Tirupur City for isolating the PA strain from blood sample and confirmed the bacterial strain using standard test method followed by drug susceptibility test for identification of resistant’s. In this study the maximum resistant was recorded in antibiotics Vancomycin 99% and maximum sensitive was observed in antibiotic Levofloxacin 99%. The blood stream infection causing PA plasmid extracted in 19 carcinogenic isolates which was showed more than 50% resistance selected for plasmid isolation. Due to the ability of resistant mechanism present in BSI pathogen the identification of new resources for novel antibiotics used urgently need to carry out a specific task to deal the blood stream infection. Moreover the enzyme especially the amylase have been isolated and purified from marine sea cucumber associated microorganism and the antimicrobial assay was done by using well techniques. The maximum zone of inhibition at 50, 100 and 150µl (, 25mm, 26mm and 16mm, 22mm, 23mm) of amylase extract was observed in the PAVG43,60 isolates. So hence this motto of this research proved the sea cucumber associated extracted amylase enzyme play a major role in the diagnosis of blood stream infection in cancer patients. Background This study describes the disease burden, clinical characteristics, antibiotic management, impact of multidrug resistance and outcome of Pseudomonas aeruginosa bloodstream infection (PABSI) among children admitted to a tertiary referral hospital for children in Cape Town, South Africa. Methods A retrospective descriptive study was conducted at a paediatric referral hospital in Cape Town, South Africa. Demographic and clinical details, antibiotic management and patient outcome information were extracted from medical and laboratory records. Antibiotic susceptibility results of identified organisms were obtained from the National Health Laboratory Service database. Results The incidence risk of PABSI was 5.4 (95% CI: 4.34–6.54) PABSI episodes / 10,000 hospital admissions and the most common presenting feature was respiratory distress, 34/91 (37.4%). Overall, 69/91 (75.8%) of the PA isolates were susceptible to all antipseudomonal antibiotic classes evaluated. Fifty (54.9%) of the PABSI episodes were treated with appropriate empiric antibiotic therapy. The mortality rate was 24.2% and in multivariable analysis, empiric antibiotic therapy to which PA isolates were not susceptible, infections present on admission, and not being in the intensive care unit at the time that PABSI was diagnosed were significantly associated with 14-day mortality. Conclusions PABSI caused appreciable mortality, however, appropriate empiric antibiotic therapy was associated with reduced 14-day mortality.
