Background: Psoriasis is extensively studied among middle-aged adults, but not many have studied psoriasis in older adults(geriatrics). Objectives: To analyze epidemiological, clinical, comorbidities and therapeutic profile of geriatric psoriasis(GP). Methods: All consenting clinically diagnosed psoriatic patients ≥60 years were divided into two groups: Elderly psoriatic(EP)(60-75 years) and Ultra elderly psoriatic(UP)(>75 years). The dermatologist filled their clinical characteristics' standardised questionnaire to determine comorbidities, drug interactions profile and compared with age-matched controls using Chi square test. Results: Prevalence of GP is 14.5%(Average age:68 years; Sex ratio=2.5:1) among geriatrics attending dermatology OPD. Most common(49.9%) as well as initial site affected(39.3%) were Palms±Soles. Nummular plaque (90.2%) was most common type. Superficial fungal infection(26.9%) and pruritus(90.8%) were commonly associated cutaneous disease and symptom(p<0.05) respectively. Hypertension(49.7%), Diabetes Mellitus(22.8%), dyslipidemia(25.8%) and Metabolic syndrome(MS)(17.8%) were associated systemic comorbidities. Dyslipidemia and MS(p<0.0001) were more common among GP(163) than controls(963). Drug aggravated psoriasis could not be linked to polypharmacy. Although, topicals were mainstay, oral Methotrexate was most efficacious systemically. Conclusion: Our findings suggest a trimodal age of psoriasis onset at 68 years. As age advances, psoriasis severity decreases, unstable guttate lesions decrease; palms±soles commonly involved; infrequent familial occurrence with Polypharmacy not aggravating psoriasis. Active screening for cardiovascular comorbidities in all geriatric psoriasis patients is highly recommended.

Psoriasis is an immune‐mediated chronic, relapsing, inflammatory, systemic disease induced by a combination of genetics and environment. Currently, there are limited reports on the epidemiological and clinical characteristics of geriatric psoriatic patients in mainland China. This study analyzed the epidemiological characteristics, clinical features, and comorbidity rates of geriatric patients with psoriasis and evaluated the influence of age of onset on disease characteristics. This retrospective study enrolled 1259 geriatric patients with psoriasis in hospitals affiliated with the National Standardized Psoriasis Diagnosis and Treatment Center in China from September 2011 to July 2020 to analyze the epidemiological characteristics, clinical features, and prevalence of comorbidity in geriatric psoriasis. The cases were classified according to the age of onset into two groups to compare differences: early‐onset psoriasis (EOP) and late‐onset psoriasis (LOP). The mean age of geriatric patients with psoriasis was 67, with a 1.8:1 male‐to‐female ratio and 10.7% positive family history. The clinical manifestations of plaque psoriasis accounted for a high proportion (82.0%) and 85.1% of patients had moderate to severe disease. Overweight (27.8%), hypertension (18.0%), joint involvement (15.8%), diabetes (13.7%), and coronary heart disease (4.0%) were the first five common comorbidities. The LOP group had significantly more patients (79.9%) than the EOP group (20.1%). Positive family history was significantly associated with the EOP group (21.7%) than the LOP group (7.9%). The scalp (60.2%) was the most affected area, followed by the nails (25.3%), palmoplantar region (25.0%), and genitals (12.7%). This study analyzed the epidemiological and clinical characteristics of geriatric psoriasis in China and found that age of onset had no effect on disease characteristics or other comorbidities, except for toenail involvement, diabetes, and joint damage.

Aims Psoriasis is a common immune-related skin disorder that affects 2-3% of worldwide people. This study aimed to determine the disease characteristics of psoriasis patients. Materials & MethodsIn this descriptive cross-sectional study, using the convenience sampling method, 164 psoriatic patients who were referred to a dermatology clinic in Yasuj from 2013 to 2014 participated. Demographic information and medical parameters were collected by a dermatologist through observation and using a researcher-made checklist. SPSS software version 19 was used for the statistical analysis of data. Findings Participants were consisting of 106 males and 58 females. The mean age of females was 28.72±15.89, and for males was 29.83±23.19. Most of them had the mild disease (48.3%), and common plaque-type psoriasis (87.7%). Sites of skin involvement were 80.4% on extremities. Nail involvement was 31.7% and arthritis was detected in 3% of cases. There was a history of pruritus or burning sensation in 41.5% of cases. There was only a 17.1% positive family history. Conclusion According to the results of this study, the ratio of men with psoriasis to women was higher. Most of the skin lesions caused by this disease were observed in the extremities, head, face and trunk, respectively, and in this respect both gender were similar and few participants reported a positive family history in this regard.
