Background: Zika virus infection during pregnancy has been recently associated with congenital microcephaly and other severe neural tube defects. However, the magnitude of confirmed cases and the scope of these anomalies have not been extensively documented. This review focuses on the magnitude of laboratory-confirmed congenital Zika virus cases among probable cases and describing the patterns of congenital anomalies allegedly caused by the Zika virus, information which will inform further research in this area. Methods: We conducted a literature search for English-language articles about congenital Zika virus infection using online electronic databases (PubMed/MEDLINE, POPLINE, Embase, Google Scholar, and Web of Knowledge). The search terms used were, “zika”, “pregnancy”, [year], “microcephaly”, “infants”, “children”, “neonates”, “foetuses”, “neural tube defect”, and “CNS manifestations” in different combinations. All articles reporting cases or case series between January 2015 and December 2016 were included. Data were entered into a Microsoft Excel database and analysed to obtain proportions of the confirmed cases and patterns of anomalies. Results: A total of 24 articles (11 case series, 9 case reports, and 4 others) were found to be eligible and included in this review. These articles reported 919 cases, with or without microcephaly, presumed to have congenital Zika virus infection. Of these cases, 884 (96.2%) had microcephaly. Of the 884 cases of microcephaly, 783 (88.6%) were tested for Zika virus infection, and 216 (27.6%; 95% confidence interval, 24.5% to 30.8%) were confirmed to be Zika virus-positive. In addition to microcephaly, other common abnormalities reported – out of 442 cases investigated – were calcifications of brain tissue (n=240, 54.3%), ventriculomegaly (n=93, 20.8%), cerebellar hypoplasia (n=52, 11.7%), and ocular manifestations (n=46, 10.4%). Conclusion: Based on the available literature, Zika virus infection during pregnancy might lead to a wide array of outcomes other than microcephaly. There is a need for more epidemiological studies in Zika-endemic areas, particularly in Africa, to ascertain the role of Zika virus in causing congenital neurological defects.

Síndrome de Guillain-Barré concomitante con infección por virus Zika Resumen OBJETIVOS: Comunicar una serie de casos de síndrome de Guillain-Barré y describir las características clínicas de una posible asociación con infección previa por virus Zika. MATERIAL Y MÉTODO: Estudio descriptivo de serie de casos en el que se recolectaron datos clínicos, bioquímicos y demográficos de los pacientes con síndrome de Guillain-Barré tratados en el Hospital General Acapulco con antecedente reciente de síndrome viral agudo sospechoso de infección por virus Zika, del 24 de julio al 24 de agosto de 2016, con realización de estudio de líquido cefalorraquídeo y comprobación de disociación albúmino-citológica. Se estableció diagnóstico de certeza de síndrome de Guillain-Barré según los criterios de Asbury-Cornblath y Brighton. RESULTADOS: Se estudiaron 10 pacientes con síndrome de Guillain-Barré y antecedente reciente de síndrome viral agudo compatible con infección por virus Zika; la edad media de los sujetos en estudio fue de 47.8 años, 4 eran hombres. Los déficits motor y sensitivo sobrevinieron en una mediana de 4.5 y 4.3 días, respectivamente, después del inicio de los síntomas de infección viral. Hubo complicaciones, como neumonía asociada con ventilación mecánica, infección de vías urinarias en tres casos y disautonomía en dos casos. CONCLUSIONES: Todos los casos de este brote de síndrome de Guillain-Barré tenían antecedente reciente de infección sospechosa por virus Zika, lo que refuerza la evidencia existente de la asociación entre el síndrome de Guillain-Barré y la infección por virus Zika.

Introduction: Although Zika virus cases have been reported in Colombia since 2015, its clinical and pharmacological characteristics have not yet been described.Objective: To describe the main clinical manifestations and sociodemographic characteristics of patients diagnosed with Zika and the treatment provided to them at a primary care hospital in a municipality of Colombia.Materials and methods: Descriptive cross-sectional study. The study population consisted of patients diagnosed with Zika between January 1 and July 25, 2016 at a first-level hospital. Sociodemographic, clinical and pharmacological variables, as well as adverse clinical and paraclinical outcomes associated with the infection were included. Descriptive statistics were performed. A χ² test was used for categorical variables, and a multivariate analysis was conducted using Epi info 7.1 software.Results: 254 individuals infected with Zika virus during the study period were identified, and Zika diagnosis was more frequent in women (68.5%). Regarding treatment, 90.9% of the cases were treated using acetaminophen. The most commonly reported symptoms were rash (81.1%) and pruritus (55.9%). In addition, antihistamines were the most frequent comedication (31.9%). Factors such as being a woman, being pregnant and inpatient treatment were associated with adverse clinical and paraclinical outcomes.Conclusion: The clinical manifestations described here are similar to those reported in other populations. Furthermore, inappropriate pharmacological management practices that can lead to complications in this population, such as bleeding, were observed in some cases, thus educational interventions on the proper prescription of medications for treating this disease aimed at general physicians working in Zika affected areas must be implemented in order to improve the prognosis of these patients.
