Eosinophilic Esophagitis

Abstract: Eosinophilic esophagitis is a clinicopathological disease characterized by GERD-like symptoms, food impaction and dysphagia that occur with the histological finding of dense esophageal eosinophilia. Endoscopic features include linear furrows, white exudate and longitudinal shearing. Current concepts regarding the pathophysiology relate to a food allergen-induced overexpression of eosinophil-related cytokines including Il-5, IL-13 and eotaxin-3. Effective treatments include topical and systemic corticosteroids … Show more

“…In the esophagus, these approaches are required to establish the existence and identity of stem cells and to characterize their self-renewal and multipotency properties. This is critical for our understanding of the cellular basis of widely prevalent esophageal diseases, such as acid reflux-induced esophagitis, eosinophilic esophagitis, and esophageal carcinoma (18,19).…”

“…As a consequence of intense research in the field of esophageal eosinophilia, our understanding of eosinophilic esophagitis (EoE) has developed from strict clinic-pathologic criteria leading toward a conceptual definition which includes pathogenic aspects (1)(2)(3). According to current recommendations, EoE represents a chronic, immune/antigen-mediated esophageal disease characterized clinically by symptoms of esophageal dysfunction and histologically by eosinophil-predominant inflammation (2).…”

“…Children generally present with non-specific symptoms of GERD, abdominal pain or failure to thrive; other present with dysphagia and esophageal food impaction [4] . The entire esophagus is often infiltrated with increased number of eosinophils [> 15 to 20 eosinophils per high power field (HPF)] a differential feature from the increased number of eosinophils found occasionally in the distal esophagus in patients with GERD of less than 15 eosinophils per HPF [16,17] . A recent study demonstrated that a cutoff value of at least 15 eosinophils per high-power field has a sensitivity of 100% and specificity of 96% for establishing the histologic diagnosis of eosinophilic esophagitis, irrespective of location of the sample (proximal or distal esophagus).…”
