Objective: To assess the relationship of air pollution and hematologic parameters in a population-based sample of children and adolescents. Methods:This cross-sectional study was conducted in 2009-2010 among school students randomly selected from different areas of Isfahan city, the second largest and most air-polluted city in Iran. The association of air pollutants levels with hemoglobin, platelets, red and white blood cells (RBC and WBC, respectively) levels was determined by multiple linear and logistic regression analyses, after adjustment for age, gender, anthropometric measures, meteorological factors, and dietary and physical activity habits. Results:The study participants consisted of 134 students (48.5% boys) with a mean age of 13.10±2.21 years. While the mean Pollutant Standards Index (PSI) was at moderate level, the mean particulate matter ≤ 10 µm (PM 10 ) was more than twice the normal level. Multiple linear regression analysis showed that PSI and most air pollutants, notably PM 10 , had significant negative relationship with hemoglobin and RBC count, and positive significant relationship with WBC and platelet counts. The odds ratio of elevated WBC increased as the quartiles of PM 10 , ozone and PSI increased, however these associations reached to significant level only in the highest quartile of PM 10 and PSI. The corresponding figures for hemoglobin and RBC were in opposite direction. Conclusions:The association of air pollutants with hematologic parameters and a possible pro-inflammatory state is highlighted. The presence of these associations with PM 10 in a moderate mean PSI level underscores the necessity to re-examine environmental health policies for the pediatric age group.J Pediatr (Rio J). 2011;87(4): Air pollution, cell blood count, atherosclerosis, inflammation, children, prevention. Métodos:Este estudo transversal foi realizado em 2009-2010 com estudantes escolhidos aleatoriamente de diversas áreas de Isfahan, a segunda maior e mais poluída cidade iraniana. A associação entre os níveis de poluentes do ar e os de hemoglobina, plaquetas, glóbulos brancos (GB) e glóbulos vermelhos (GV) foi determinada pelas análises linear múltipla e de regressão logística ajustadas para idade, sexo, medidas antropométricas, fatores meteorológicos, e hábitos alimentares e de atividade física. Increasing urbanization across the globe, combined with an increased use of chemicals in various regions, contributes to several environmental events that influence environmental health. Measures that identify environmental factors and events should be introduced to facilitate epidemiological investigations by health services. The Brazilian Ministry of Health published a new list of notifiable diseases on 25 January 2011 and introduced environmental events as a new category of notifiable occurrences. The Center for Epidemiologic Surveillance in State of Sao Paulo, Brazil, created an online notification system that highlights “environmental events”, such as exposure to chemical contaminants, drinking water with contaminants outside of the recommended range, contaminated air, and natural or anthropogenic disasters. This paper analyzed 300 notifications received between May 2011 and May 2012. It reports the number of notifications with event classifications and analyzes the events relating to accidents with chemical substances. This paper describes the characteristics of the accidents that involved chemical substances, methods used, types of substances, exposed population, and measures adopted. The online notification of environmental events increases the analysis of the main events associated with diseases related to environmental chemicals; thus, it facilitates the adoption of public policies to prevent environmental health problems.

Objective: to present good practice guiding standards in nursing care to children intoxicated by household cleaners. Method: study with documentary nature, using narrative review, with literatures of technical-scientific evidence on the subject and protocols of a center for toxicological information in the northwestern region of Paraná. The good practice guiding standards were systematized and described in seven axes. Results: the procedures of reception related to family and obedience to ethical and relational aspects of care, primary assessment and measures for life support, nursing history and clinical epidemiological and laboratory anamnesis, decontamination measures, use of antidotes, symptomatic treatment and complementary exams and nursing guidelines for hospital discharge, regarding the structure and care process. Conclusion: adapted to the structural conditions and welfare services, this study may contribute to improve the quality of care and a more agile, efficient and complete care to the intoxicated person. Conclusão: espera-se que, adaptado às condições estruturais e assistenciais dos serviços, este estudo possa contribuir para melhorar a qualidade da assistência e para que o atendimento ao intoxicado se torne mais ágil, eficaz e completo. Las normas rectoras de la buena práctica fueron sistematizadas y descritas en siete ejes. Resultados: los procedimientos para la recepción refieren a la familia y la obediencia a los aspectos relacionales y éticos de la atención primaria, la evaluación y las medidas de apoyo a la vida, la historia de la enfermería y la anamnesis clínica, epidemiológica y de laboratorio las medidas de descontaminación, uso de antídotos, tratamiento sintomático y exámenes complementarios y de enfermería al alta hospitalaria, directrices sobre la estructura y el proceso de cuidados. Conclusión: adaptado a las condiciones estructurales y de servicios de bienestar, este estudio puede contribuir a mejorar la calidad de la atención y para un servicio al intoxicado más ágil, eficiente y completa.
