We conclude that patients with hip replacement show more improvement 3 months after the surgery than patients with knee replacement. Furthermore, it was seen that patients who were more disabled before surgery achieved more improvement than the less-disabled patients. However, they did not achieve the level of the less disabled patients. Thus, the recommendation that total joint replacement should be performed as late as possible should be reviewed.

Zusammenfassung ! Hintergrund: Die Überprüfung von Ergebnissen der Medizinischen Trainingstherapie (MTT) kann aus biomedizinischer Sicht über Kraftmessungen sowie aus subjektiver Sicht des Trainierenden erfolgen. Spezifische Instrumente zur Selbstbeurteilung der Effekte in der MTT liegen gegenwärtig nicht vor. Ziel: Ziel der Untersuchung war es, einen Fragebogen (META) zur Erfassung von Ergebnissen in der MTT aus der subjektiven Sicht der Patienten zu entwickeln sowie auf seine Reliabilität, inhaltliche Dimensionierung und Praktikabilität zu überprüfen. Methode: In einer Voruntersuchung wurden über 29 explorative Interviews mögliche Effekte der MTT abgefragt. Die inhaltlich kategorisierten Ergebnisse wurden in standardisierte Fragen überführt und in einer Hauptuntersuchung in 4 ambulanten Rehabilitationszentren hinsichtlich Reliabilität, Faktorenbildung und Praktikabilität überprüft. Ergebnisse: Die 181 Untersuchungsteilnehmer waren im Mittel 48 Jahre alt (SD ± 19). Die mittlere Trennschärfe des META lag bei 0,5 und die mittlere Item-Schwierigkeit bei 0,7. Der Cronbach Alpha-Koeffizient wies mit 0,9 eine hohe interne Konsistenz auf. Die Test-Retest-Reliabilität ergab über den Intraklassen-Korrelationskoeffizienten (ICC) eine sehr hohe Übereinstimmung von über 0,9. Eine Faktorenanalyse zeigte 6 Faktoren, wobei der 1. Faktor 29 % der Varianz erklärte. Schlussfolgerung: Das META stellt ein praktikables Selbstbeurteilungsverfahren dar, das in Ergänzung zu objektiven Kraftmessungen zum Einsatz kommen kann. Änderungssensitivität, Konstruktund Kriteriumsvalidität sollten noch bestimmt werden.Abstract ! Background: The results of medical exercise therapy (MTT) can be tested by maximum strength tests from a biomedical point of view as well as from the patients' own subjective point of view. To date specific patient-based evaluation instruments for MTT effects are not available. Objective: The aim of this study was to develop a patient questionnaire (META) to evaluate the effects of medical exercise therapy from the patient's subjective point of view. The questionnaire was tested in relation to its reliability, its factual analysis and its practicability. Method: As preliminary research, 29 interviews were carried out to ascertain possible MTT effects. The categorised results were transferred into standardised questions. In a cross-section survey in 4 out-patient rehabilitation centres their reliability, factor analysis and practicability were tested. Results: The mean age of the 181 patients was 48 (SD ± 19). The META's item selectivity amounted to 0.5 and the mean item difficulty to 0.7. Cronbach Alpha reached 0.9 and therefore showed high internal consistency. The test-retest reliability highly coincided with the intraclass correlation coefficient (ICC) of over 0.9. The factor analysis identified 6 factors, of which the first one accounted for 29 % of the variation. Abstract: The XSMFA-D could be demonstrated to be an appropriate short questionnaire for the evaluation of therapy results from patient's perspective. The use of the XSMFA-D can be recommended for routine use. Further investigations of the instrument will be undertaken.
