Entrapment of diagnostic catheter within Advisor HD grid mapping catheter

Abstract: The potential for catheter entanglement with the HD Grid mapping catheter is explicitly stated in the manufacturer's product manual. A case of an entrapped 6 French quadripolar diagnostic catheter within an HD Grid mapping catheter is presented. We discuss the diagnosis, management, and resolution of this complication in our patient. The patient's arrhythmia was successfully eliminated, and no vascular complication in the postprocedural setting nor arrhythmia recurrence at follow‐up were observed. Strategies t… Show more

“…Notably, there are no reports of similar issues with the similarly designed Advisor HD Grid (Abbott, Abbott Park, IL) mapping catheter, although there are reports of entanglement of this catheter with other diagnostic catheters. 14 The risk of entanglement with the Impella can be avoided by using the Impella 5.5 (Abiomed, Danvers, MA) ( Figure 2 D), which does not have a pigtail. The Impella 5.5 can provide an additional support over the Impella CP (up to 5.5 L/min), but requires a surgical axillary artery cutdown or transcaval approach.…”

“…There have been previous reports of device entrapment during catheter ablation of AF. Device entrapment can occur through interaction with other instruments used during the procedure 1 , 2 or with the cardiovascular structures themselves. While rare, the most common concern is entrapment of circular mapping catheters (eg, the Lasso catheter [Biosense Webster]) in the mitral apparatus.…”

“…The safety of the fluoro-less or near-zero fluoroscopic procedures has been proven; however, there have been a few reports of some complications that could have been prevented if fluoroscopy was used. A case report on the entrapment of a diagnostic mapping catheter within an Advisor HD grid catheter [ 26 ] as well as severe pulmonary regurgitation secondary to ablation in the proximal main pulmonary artery (MPA) to ablate the focus of PVCs that appeared to be from right ventricular outflow tract (RVOT) [ 27 ] highlight the need for the availability of fluoroscopy during the performance of fluoro-less or near-zero fluoroscopic procedures.…”
