A radiografia convencional é o método de imagem mais acessível, seja pelo menor custo, seja pela maior disponibilidade. Não apresenta alta sensibilidade ou especificidade, de forma geral é útil no contexto clínico para avaliação das artropatias e do dano estrutural das entesopatias, em particular na avaliação do processo de neoformação óssea pós-inflamatória das ênteses. A ossificação das ênteses ocorre como processo de reparo em reação a uma inflamação prévia que ocorreu de maneira geral alguns meses antes do aparecimento do achado radiográfico, conforme descrito nos artigos “Êntese – Uma R(E)volução” e “Fisiopatologia das entesopatias: aspectos mecânicos e inflamatórios (axial e periférico)” desta revista. Portanto, os achados da radiografia convencional não demostram o processo inflamatório atual, mas sim a sequela e reparo de um processo pregresso. Os locais mais comumente afetados podem ser avaliados pela radiografia, sendo por óbvio os locais onde temos ênteses mais submetidas a estresse mecânico e inflamatório. Quando as articulações periféricas são acometidas, radiografias das mãos e dos pés são úteis para este tipo de avaliação. Os sítios mais comumente pesquisados são calcâneos, ombros, joelhos e bacia. A avaliação das ênteses na região das articulações da coluna vertebral pode corroborar no diagnóstico destas enfermidades. Os padrões de calcificação ou neoformação óssea podem ser úteis na diferenciação das entesopatias. Calcificações com padrão de “tração”, delgadas e lineares, sem perda do contorno anatômico, ou depósitos difusos e calcificações tendíneas, ou com formações “em escamas” e associados a erosões, respectivamente sugestivos de processo mecânico-degenerativo, doenças por depósito e inflamação propriamente dita. Outro ponto a ser considerado na avaliação destes pacientes são possíveis variações anatômicas que acarretam alterações biomecânicas e por consequência podem afetar o padrão e a frequência de aparecimento das alterações radiográficas nas ênteses, como por exemplo, pé cavo, pé plano, escolioses, dismetria dos membros, entre outras. Adicionalmente, com o envelhecimento, ocorre naturalmente degeneração nas ênteses dos tendões, ligamentos e fáscias que se apresentam como proliferação óssea frequentemente identificadas nas radiografias, sem relação obrigatória com manifestação clínica. Em resumo, as radiografias convencionais são úteis no raciocínio diagnóstico das entesopatias, com atenção às características clínicas, anatômicas e funcionais dos indivíduos. Unitermos: Entesopatia. An enthesis refers to the interface at which a tendon or a ligament integrates into the periosteum. Its morphology can be influenced by intrinsic factors such as sex, age, and extrinsic factors such as levels of activity, which will in turn impact on bone remodeling and lead to morphological changes. In bioarcheology, entheseal changes have had a long tradition of being used for the reconstruction of past activities. The literature has shown that in some cases of osteoarthritis, entheseal changes are associated with osteoarthritic manifestations. This work aims to evaluate the relationship between the degree of entheseal changes and the severity of osteodegenerative processes. The studied materials consisted of 30 humeri and 30 femora from the osteological collection at St George's University of London. Intensities of both entheseal changes and osteodegenerative processes were macroscopically assessed and scored.The difference in scores of entheseal changes between osteoarthritic groups and nonosteoarthritic groups is statistically significant at a confidence level of 95% (α = .05) for both the humeri and femora. Results show a positive correlation between the degree of entheseal changes and the severity of osteodegenerative processes in the femora, suggesting that enthesis may play a role in osteoarthritis. Findings from this work supports the proposed hypothesis that the degree of entheseal changes and the severity of osteoarthritic manifestation are related. This work contributes to current knowledge that osteoarthritis is a disease involving the whole joint; the enthesis could potentially be a target for the diagnosis of osteoarthritis.

The aim of this review is to describe the metabolism of calcium in ankylosing spondylitis compared to physiologic conditions, and to present the current evidence on the benefits and disadvantages of calcium supplementation in these patients. A narrative review of the literature was conducted using the PubMed database and a total of 65 articles were selected. Calcium is involved in many physiopathological processes, including inflammation, bone loss and bone formation, all of which occur in ankylosing spondylitis. Many ankylosing spondylitis patients suffer from concomitant osteopenia or osteoporosis, which represent indications for calcium supplementation. Conversely, there are still concerns about the use of calcium salts for the prevention of bone fragility in non-osteoporotic or non-osteopenic patients. <br /> In these cases, biologic agents may indirectly normalize calcium dysmetabolism by rebalancing the cytokine milieu, in turn associated with bone remodeling. Calcium supplements may be disadvantageous for entheseal calcifications, but so far there are no clear data confirming that such an association exists.
