Purpose To assess the stiffness of the natural human internal limiting membrane (ILM) and evaluate potential changes of the mechanical properties following staining with brilliant blue (BB) and indocyanine green (ICG). Methods Unstained ILM specimens were obtained during ophthalmic surgical procedures. After removal, the specimens were dissected into five parts. Two fragments were stained with BB and ICG, respectively, for 1 min, another two specimens were stained similarly followed by additional subsequent illumination using a standard light source (PENTA LUX x 50, Ophthalmologische Systeme GmbH Fritz Ruck). The fifth part served as an untreated control. All specimens were then analyzed using atomic force microscopy (AFM) in contact mode with a scan rate of 0.6 Hz. Two scan regions of 10 Â 10 mm were chosen and stiffness was determined by using AFM in a force spectroscopy mode. The force curves were plotted with a data rate of 5000 Hz. In all specimens both the retinal side and vitreal side were analyzed.

Aim. A retrospective analysis of the results of surgical treatment of large idiopathic macular holes depending on the technique of surgical intervention. Methods. The results of surgical treatment of 60 patients (60 eyes) with idiopathic macular holes with a diameter of more than 800 μm were studied in the Republican clinical ophthalmology hospital of Kazan. The average age of the patients was 66.2±5.37 (61-74) years. The patients underwent complex ophthalmological examination before the surgery and 10 days and 1 month after the surgery, including visometry, tonometry and optical coherence tomography. Patients were divided into two groups (30 subjects each), comparable in clinical and epidemiological parameters: group 1 - standard surgical tactics, group 2 - surgical treatment according to the method proposed by professor A.N. Samoylov. Visual acuity with correction before surgery in group 1 was 0.11±0.05, in group 2 - 0.12±0.06. Results. Ten days after the surgery, group 1 had anatomical closure in 23 (76.7%) patients, anatomical result was not achieved in 7 (23.3%) patients. In group 2, complete closure of the rupture was achieved in 27 (90.0%) patients, incomplete - in 3 (10.0%) patients. In 1 month in group 1, complete closure of the macular rupture was observed in 22 (73.3%) patients, in 8 (26.7%) patients the result was not achieved. In group 2, complete closure of the rupture was determined in 28 (93.3%) patients, incomplete closure - in 2 (6.7%) patients. Visual acuity 1 month after the surgery in group 1 was 0.15±0.08, and in group 2 - 0.32±0.11 (p <0.05). Conclusion. Surgical treatment of large idiopathic macular holes according to the modified technique of the inverted internal limiting membrane flap proposed by professor A.N. Samoylov, provides better anatomical and functional results in comparison with the standard technique (p <0.05).

Background and Objectives: To explore the potential of Er:YAG-laser irradiation for precise and tractionless retinal tissue and inner limiting membrane ablation. Materials and Methods: We used free-running Er:YAGlaser irradiation (l ¼ 2.94 mm) transmitted either through a 10 cm long low-OH-quartz fiber or a 2 m long sapphire fiber that produced a more homogenous light distribution at the fiber tip. Retinal ablation in porcine retinal explants was performed under air or perfluorodecaline (PFD). Ablation depth was evaluated by optical coherence tomography (OCT) and from histologic sections. Results: A radiant exposure of 5.0 J/cm 2 delivered through a low-OH-quartz fiber and PFD caused a complete transsection of the neurosensory retina. Radiant exposures between 3.5 and 2.0 J/cm 2 resulted in marked variations of ablation depth and adjacent thermal damage. By contrast, laser pulses of 4.0 and 3.0 J/cm 2 transmitted through the sapphire fiber produced more homogenous defect patterns and less thermal damage. Close to the ablation threshold, with 1.0-2.0 J/cm 2 , ablation was limited to a 10-20 mm thin layer of the neural retina. Conclusions: We achieved in vitro ablation of inner retinal layers, but could not produce selective and reproducible ILM removal.
