Enterocutaneous Fistula and Ventral Hernia after Absorbable Mesh Prosthesis Closure for Trauma: The Plain Truth

Abstract: Although the severity of abdominal injury is the most important factor for fistula formation, the most important factor for ventral hernia development is the duration of AMPC. Daily interventions, such as mesh tightening, may be necessary to limit ventral hernia in these high-risk patients.

“…Especially the rate of enterocutaneous fistulae is much higher in our collective of patients than reported in previous studies [4,10,20]. This indicates that a promising low rate of enterocutaneous fistulae shown by others can be likely put down to a high percentage of trauma patients without peritonitis and a fascial closure within the first days.…”

“…Compared to promising results in trauma patients showing a total fascial closure rate of 65–88% and a late fascial closure rate (after more than 7 days) of up to 48% [3,8,10,17,20], our results indicate that a fascial closure rate might neither be a result of the selective collective of patients nor of the surgical dressing alone, but also of severity of the underlying disease [10]. However, independent from the disease, a fascial closure within the first days is of prognostic value and should be aspired to whenever possible [4]. …”
