Objectives: To find out the disease pattern of patients attended at the ENT outpatient department of a tertiary level hospital in the center of Dhaka city and to aware the people about the consequences of ENT diseases, necessity of early management, prevention and also the health care management about the necessity of establishment of proper referral system. Methodology: It is retrospective observational study performed in the ENT OPD of Shaheed Suhrawardy Medical College Hospital based on data collected by Resident Surgeon during his consultation of referral patients in his OPD chamber between 01 July 2014 and 31 December 2014. A total of 3686 patient's data were analyzed. Results: Out of 3686 OPD patients male were 1947 (52.82%) and female 1739 (47.17%), ratio is 1.12:1. 11 to 40-year age group patients were more, among them 11-20 (17.25%), 21-30 (21.51%) and 31-40 (21.62%). 47.97% were presented with Ear diseases, 18.96% with Sinonasal diseases, 19.89% with Throat diseases, 2.03% with Laryngeal & Hypopharyngeal diseases, 9.44% with Neck diseases and 1.71% with Oral cavity problems. Conclusion: Prevalence of Ear diseases are more in Bangladesh like other developing countries. A good numbers of cancer patients were fund in the study. Almost half of the patients were attended from rural and slum areas, means under privileged people are getting concern about ENT problems like other medical diseases. A large number of patients came to this center which can be easily managed in Union sub-center, Upzilla health complex or District general hospital. It is recommended to develop proper referral system in our health system.

Background: A retrospective descriptive study was made to find out the disease pattern, regional mobility, co-morbid details and malignancies related to otorhinolaryngological disorders.Methods: Consecutive patients admitted to male and female wards over a period of 24 months commencing from May 2016 to April 2018 to a rural teaching hospital formed the materials. Their socio-demographic details, nature of illnesses, clinical course, final diagnosis and interventions made were collected from the case records and analyzed. Results: During the study period 1740 cases were admitted; 854 (49.4) males and 886 (50.9%) females. Among them 642 (34.9%) suffered from otological disorders, and the rest had, rhinological related (607; 34.9%), laryngeal related (321; 18.4%) and other illnesses (170; 9.8%). The region wise data were also analyzed and recorded. Among the malignancies, oropharyngeal dominated followed by laryngopharyngeal and laryngeal. Among the in-patients studied, ear diseases were more common and these were followed by nose and throat diseases.Conclusions: It is suggested to train primary care doctors and other health care workers on common ENT disorders to provide attention and care for emergency cases and refer the deserving cases in time to higher centres for appropriate care and management.

An Audit of Referrals to the Minor Operating Theater of the Patient Presenting in Otolaryngology OPD with Ear Discharge

Introduction: Despite a larger frequency of OPD patients requiring treatment, why is it not done in OPD should be addressed. Because there is no suction equipment in the ENT OPD, these patients who require suctioning are referred to ENT Emergency Room. This is a source of undue inconvenience for patients. This not only poses a risk to patient safety, but it also has major financial consequences, as well as a negative impact on patient satisfaction, which is a key indicator of treatment quality. Methods: This is a closed-loop audit cycle beginning with recognition of the problem and assessing the magnitude of the problem. Based on the burden of referrals to the MOT, the auditors proposed the changes in practice under hospital OPD guidelines enacted by Pakistan Otolaryngology Association. Results: A significant percentage of 52.5 is requiring ear suction in the first audit cycle. The percentage of patients that had to be referred to ENT OT decreased from 52.5% in the previous audit to a significantly low value of 9.5% in this audit. This clinic’s role is to decrease the burden of referrals, reduce patients’ misery of going to the ER for suctioning, and improve patient satisfaction. Conclusion: Our study discerns the fact that the implementation of some simple protocols will lead to an immense change. The resultant reduction in clinical activity will lead to a reduction in the workload of the doctors too.
