Ensuring uninterrupted supplies of antiretroviral drugs in resource-poor settings: an example from Malawi

Abstract: Problem Drug procurement and distribution practices are weak in many resource-poor countries, and are a major reason for lack of access to medicines. With many countries scaling up antiretroviral therapy (ART), it is vital to avoid interrupted drug supplies, which would lead to drug resistance and treatment failure. Approach Malawi has adapted a model, based on that adopted by the country's Tuberculosis Control Programme, to allow rational ART drug forecasting. Local setting The model includes a focus on one s… Show more

“…Furthermore, high rates of nevirapine and efavirenz crossover may hinder drug forecasting at the program level. In sub-Saharan Africa, drug forecasting and stable drug management systems are crucial to ensuring the success of HIV treatment programs [20]; reductions in rates of drug switches could contribute to the success of ART delivery programs.…”

“…20,21 Without a robust system in place there will inevitably be drug and commodity ruptures as ART procurement and distribution will be dramatically increased from thousands to millions. There is also the issue of ensuring and monitoring drug adherence in individuals who generally feel well.…”
