Ensuring reliability of outcome measures in multicenter clinical trials of treatments for acute ischemic stroke. The program developed for the Trial of Org 10172 in Acute Stroke Treatment (TOAST).

Abstract: Ensuring the reliability and validity of outcome measures used in clinical trials is essential to the success of the trial. The Trial of Org 10172 in Acute Stroke Treatment (TOAST) is a multicenter clinical trial that is recruiting patients with acute ischemic stroke seen at medical centers across the United States. This paper describes an approach to train physicians to use three clinical measures: the National Institutes of Health (NIH) Stroke Scale, a supplemental motor examination, and the Glasgo… Show more

“…Mesmo em estudos com pacientes apre s e n t a d o s em videotape, em que a variabilidade natural do quadro clínico está eliminada, estes itens foram problemáticos 28,30 . Possivelmente sua própria natureza im-põe dificuldades, como é atestado por estudos avaliando a concordância na execução do exame neurológico clássico 3 1 .…”

“…4 It is a reliable tool that physicians and even nonphysicians can use to assess the clinical status of patients with stroke. 5 Because the NIHSS examination takes fewer than 8 minutes to administer, has good interrater reliability with proper training, [6][7][8][9] and is a valid prognostic indicator, its clinical application has expanded to quantify deteriorating neurologic status. 2 Neurologic deterioration (ND), an event that occurs in about one-third of patients with acute ischemic stroke (AIS) and contributes to morbidity and mortality, 10,11 has no standardized definition.…”
