Enigma: infection or allergy? Vancomycin-induced DRESS syndrome with dialysis-dependent renal failure and cardiac arrest

Abstract: A man aged 73 years with infective endocarditis presented with septic shock and was started on immediate antimicrobial therapy. His blood culture yielded no organism. Subsequently, he developed a severe allergic reaction to prolonged empirical vancomycin therapy. This manifested as fever, widespread maculopapular rash and severe progressive acute kidney injury ultimately requiring dialysis. In the context of eosinophilia, this was determined to be drug reaction with eosinophilia and systemic symptoms (DRESS) s… Show more

“…Significant hypersensitivity reactions from vancomycin have been reported, including Linear IgA Bullous Dermatosis, DRESS, SJS/TEN [ 20 ]. We performed a literature search and identified 23 cases of vancomycin induced DRESS syndrome (Table 1 ) [ 22 – 37 ]. In addition, there are two articles in French and one article in German, which are not included in the discussion here.…”

“…Table 3 and Table 4 present a summary and analysis of 44 cases of eosinophilia caused by vancomycin. 28,3565 Of these 44 cases, drug reaction with eosinophilia and systemic symptoms (DRESS), also known as drug-induced hypersensitivity syndrome, was found in 35 cases. 28,3557 In other cases, some patients had increased eosinophil counts an...…”

“…Typically, patients present with a skin rash accompanied by eosinophilia, fever, atypical leukocytosis, and multiple organ failures, including the kidneys, liver, and lungs [51,52]. Mortality rates have been documented as high as 10%, and, unlike other precipitants, vancomycin is particularly associated with an increased risk of renal involvement [53]. Seventy-four percent (74%) of DRESS cases are associated with antibiotics, with the most commonly implicated antibiotics being vancomycin (39%), β-lactams (23%), fluoroquinolones (4%), tetracyclines (4%) and sulfonamides (3%) [54].…”
