While therapeutic mesenchymal stromal/stem cells (MSCs) have usually been obtained from bone marrow, perinatal tissues have emerged as promising new sources of cells for stromal cell therapy. In this study, we present a first safety follow-up on our clinical experience with placenta-derived decidual stromal cells (DSCs), used as supportive immunomodulatory and regenerative therapy for patients with severe complications after allogeneic hematopoietic stem cell transplantation (HSCT). We found that DSCs are smaller, almost half the volume of MSCs, which may favor microvascular passage. DSCs also show different hemocompatibility, with increased triggering of the clotting cascade after exposure to human blood and plasma in vitro. After infusion of DSCs in HSCT patients, we observed a weak activation of the fibrinolytic system, but the other blood activation markers remained stable, excluding major adverse events. Expression profiling identified differential levels of key factors implicated in regulation of hemostasis, such as a lack of prostacyclin synthase and increased tissue factor expression in DSCs, suggesting that these cells have intrinsic blood-activating properties. The stronger triggering of the clotting cascade by DSCs could be antagonized by optimizing the cell graft reconstitution before infusion, for example, by use of low-dose heparin anticoagulant in the cell infusion buffer. We conclude that DSCs are smaller and have stronger hemostatic properties than MSCs, thus triggering stronger activation of the clotting system, which can be antagonized by optimizing the cell graft preparation before infusion. Our results highlight the importance of hemocompatibility safety testing for every novel cell therapy product before clinical use, when applied using systemic delivery.

ResumoIntrodução: hepatopatias crônicas figuram entre as principais causas de morte no mundo e o transplante hepático permanece como a única alternativa terapêutica curativa disponível. A terapia celular utilizando células tronco tem sido demonstrada como uma alternativa potencial, porém os mecanismos envolvidos na resolução da doença permanecem em debate. Metodologia: trata-se de uma revisão de literatura, na qual foi realizada uma consulta por artigos científicos selecionados através de busca no banco de dados do PubMed e Scielo no período de agosto de 2016 a junho de 2017. Objetivo: este estudo tem como objetivo descrever os mecanismos envolvidos na regeneração/reparo da cirrose hepática através do tratamento com células tronco mesenquimais derivadas do tecido adiposo (AD-MSCs). Resultados e discussão: os estudos utilizando terapia celular apontam como principais mecanismos a fibrinólise, imunomodulação e atividade antioxidante. Com relação a atividade fibrinolítica, o aumento proeminente na expressão de enzimas como MMP2, MMP3 e MMP9 explica a redução da fibrose normalmente observada. A atividade imunomodulatória das ADMSCs parece estar relacionada com a secreção de agentes imunossupressores e fatores que promovem a hematopoiese. Finalmente, as AD-MSCS são capazes de aumentar a atividade do NF-E2-related fator (Nrf2), um fator de transcrição crucial na expressão de diversas enzimas relacionadas à resolução do estresse oxidativo. Conclusão: os estudos utilizando a terapia celular evidenciaram que fibrinólise, imunomodulação e efeito antioxidante podem, de forma associada, contribuir para os bons resultados apresentados. Apesar dos principais mecanismos propostos terem sido apresentados neste presente trabalho, mais estudos são necessários a fim de determinar a precisa contribuição de cada um deles. Palavras-chave:

From Phenomenon to Essence: A Newly Involved lncRNA Kcnq1ot1 Protective Mechanism of Bone Marrow Mesenchymal Stromal Cells in Liver Cirrhosis

Bone marrow mesenchymal stromal cells (BMSCs) have a protective effect against liver cirrhosis. Long noncoding RNAs (lncRNAs) play crucial roles in the progression of liver cirrhosis. Therefore, it is aimed to clarify the lncRNA Kcnq1ot1 involved protective mechanism of BMSCs in liver cirrhosis. This study found that BMSCs treatment attenuates CCl4‐induced liver cirrhosis in mice. Additionally, the expression of lncRNA Kcnq1ot1 is upregulated in human and mouse liver cirrhosis tissues, in addition to TGF‐β1‐treated LX2 cells and JS1 cells. The expression of Kcnq1ot1 in liver cirrhosis is reversed with BMSCs treatment. The knockdown of Kcnq1ot1 alleviated liver cirrhosis both in vivo and in vitro. Fluorescence in situ hybridization (FISH) confirms that Kcnq1ot1 is mainly distributed in the cytoplasm of JS1 cells. It is predicted that miR‐374‐3p can directly bind with lncRNA Kcnq1ot1 and Fstl1, which is verified via luciferase activity assay. The inhibition of miR‐374‐3p or the overexpression of Fstl1 can attenuate the effect of Kcnq1ot1 knockdown. In addition, the transcription factor Creb3l1 is upregulated during JS1 cells activation. Moreover, Creb3l1 can directly bind to the Kcnq1ot1 promoter and positively regulate its transcription. In conclusion, BMSCs alleviate liver cirrhosis by modulating the Creb3l1/lncRNA Kcnq1ot1/miR‐374‐3p/Fstl1 signaling pathway.
