Enhanced Stent Imaging System Guided Percutaneous Coronary Intervention Is Linked to Optimize Stent Placement

Abstract: Stent under-expansion is a predictor of restenosis and stent thrombosis. It remains uncertain whether enhanced stent imaging (ESI) (CLEARstent) guidance can improve stent under-expansion. Our aim was to assess the effect of using ESI on stent under-expansion, after percutaneous coronary intervention (PCI) in a single center, cross-sectional observational study. Participants attending our cardiology clinic with stable angina or acute coronary syndrome, from March to September 2020 were recruited. A total of 164… Show more

“…The steps of LM PCI can be furthermore accurately controlled, especially since intracoronary imaging cannot give live insights while positioning gear inside the coronary arteries. Most importantly, the optimal positioning of the POT balloon can be guided by the contrast injection during live stent enhancement visualization and maneuvering [ 32 , 33 ]. The main advantages of stent enhancement over intracoronary imaging are its simplicity of use, short time requirements, lack of additional costs, and immediate image interpretation [ 34 ].…”

“…Particularly given that intracoronary imaging cannot offer real-time information while placing gear inside the coronary arteries, the steps of LM PCI can be more carefully controlled. Most essentially, during live stent enhancement imaging, contrast administration may be applied to direct the POT balloon into the optimal location [ 20 , 21 ]. Stent architecture assessment is also important in order to assess PCI results and complications [ 22 ].…”

“…We used ESV, a fluoroscopy-based evaluation method, as an alternative. ESV has been shown to be a reliable alternative to intravascular imaging in some studies [ 41 , 49 , 50 , 51 , 52 , 53 ]. In our study, all the lesions were evaluated for stent expansion using intravascular ultrasonography or ESV immediately after stent deployment at a nominal pressure during PCI.…”
