Enhanced Liver Fibrosis Panel as a Predictor of Liver Fibrosis in Chronic Hepatitis C Patients

Abstract: ELF panel is a good noninvasive fibrosis marker and showed similar results to TE in CHC patients. However, new cutoff points need to be established to improve its performance on patients with CHC.

“…The ELF test measures three direct markers of liver matrix metabolism in serum: hyaluronic acid (HA), tissue inhibitor of metalloproteinase-1 (TIMP-1) and propeptide of type III procollagen (PIIINP), which are combined to calculate the ELF score. The ELF score was validated for liver fibrosis assessment in a large group of patients with various CLDs (5) and has also been confirmed in specific patient cohorts (4,(6)(7)(8)(9)(10)(11). More recent studies suggest that ELF score is influenced by age, gender and liver inflammatory activity, and that these factors may confound diagnosis at earlier fibrosis stages (12,13).…”

“…In spite the initial outstanding performance of recently defined index, enhanced liver fibrosis (ELF), which combines several variables (hyaluronic acid, tissue inhibitor of matrix metalloproteinases-1, and amino-terminal propeptide of type III procollagen[41], the limited sample size of cirrhotic patients in that cohort ( n = 29) could lead to overestimate its diagnostic potential[42]. Indeed, angiopoietins ratio displays better AUC-ROCs for cirrhosis identification when ELF is analyzed in a larger cohort of patients (0.81 vs 0.78).…”

“…This was solved by using HA as part of the ELF score (43,44). The ELF score was higher in a small sample of chronic HCV patients compared to healthy controls, and it was significantly correlated with histological staging in patients with chronic HCV (43)(44)(45). Higher ELF score values were identified in older individuals.…”
