Objectives To determine the incidence of possible obstacles and patients’ characteristics when performing endovenous laser treatment (EVLT) and suggestions to improve the ability to overcome them. Method A prospective study with 215 limbs treated with EVLT. A sequence of maneuvers was performed to move the fiber forward when difficulty was found and if unsuccessful another entry site was punctured. Results In 29 of 215 (13.48%) limbs, resistance was encountered while navigating the fiber; 16 (55.17%) solved with lower limb extension; 13 (44.82%) needed ultrasound guided maneuvers; five (17.24%) needed a saline flush and in three patients (10.34%) another entry site was necessary. In 214 of 215 cases (99.53%), ablation was accomplished and no complications were observed. Conclusions Both obstruction and higher patients' BMI are the main obstacles during endovenous laser ablation and usually needs additional maneuvers to accomplish the procedure in our study and the use of tricks to overcome these issues is feasible.

Resumo: O tratamento usando a Ablação por Radiofrequência, nos últimos dez anos, tem-se mostrado promissor, com taxas de sobrevivência melhoradas. Um método alternativo no tratamento de algumas neoplasias, é modalidade técnica para as quais as opções de tratamento esgotavam-se. A Engenharia Biomédica, tradicionalmente, está associada aos avanços e intervenções em saúde e para saúde. Nesse sentido o trabalho aqui proposto discute, aplicando uma metodologia pesquisa qualitativa, identificando o impacto de desenvolver pesquisas tecnológicas por meio de confecção de dispositivos médicos. O resultado dessa análise promove uma posição implicando na necessidade de um conhecimento profundo do contexto focalizado em que patologias específicas e em quais de seus graus se propõem soluções tecnológicas. Concluise que a técnica está sendo aperfeiçoada para as mais diversas neoplasias, como Hepatocarcinoma (o mais estudado), câncer de pulmão, tumores ósseos, e até mesmo para dores crônicas de face e cabeça, cada qual com protocolos clínicos, potências, tempo e diferentes condições do paciente. Fato que nos conduz a optar com base no construtivismo social e impacto econômico por gerar estudos em Equipamentos médico-assistenciais (EMA) que representam um setor estratégico no Complexo Industrial da Saúde. Palavras IntroduçãoCiência, inovação e interdisciplinaridade para um Brasil competitivo sanará deficiências de produção tecnológica nacional. O papel da Engenharia Biomédica é o amadurecimento e a autoria decisiva nessa ação de desenvolvimento. Assim como, agregar evidências de pesquisas para guiar o desenvolvimento de um produto é uma das principais razões para que se desenvolvam estudos formadores da literatura. As revisões sistemáticas em engenharia biomédica devem conter traços para nortear o desenvolvimento de projetos, indicando novos rumos para futuras investigações e identificando as tecnologias e inovações a serem geradas. Tomemos como exemplo a tecnologia agropecuária, que em 1973, com a criação da Embrapa, tornou do Brasil uma potência agrícola, rompendo os trilhões de pensamento céticos de que "tecnologia é coisa que se compra" [1].Na literatura revisada, houve uma diversificação no uso da Ablação por Radiofrequência (RFA) na área médica. BACKGROUND: Varicose veins of the lower limbs have a high prevalence worldwide. New treatment techniques have been developed with the objectives of improving patients' quality of life and reducing recovery times. OBJECTIVE: To evaluate patients with incompetent saphenous veins treated using conventional saphenectomy or radiofrequency ablation (RF), in terms of postoperative status. METHODS: From May 2012 to April 2013 146 varicose veins patients with saphenous insufficiency, 90 of whom were treated with conventional surgery (G1) and 56 with RF ablation (G2), were evaluated prospectively. RESULTS: In G1, 88.61% of patients complained of postoperative pain and needed to take analgesics, compared with 28.85% in G2 (p<0.05). Mean pain rating on an analog scale from 0 to 10 was 3.91±2.13 points for G1 and 1.76±3.01 points for G2 (p<0.05). Recovery periods ranged from 26.63±13.3 days to 18.26±19.37 days, for G1 and G2 respectively. Mean time taken to become totally asymptomatic was 66.78±60.9 days for G1 and 38.38±46.8 days for G2 (p<0.05). CONCLUSIONS: The RF treatment method caused less postoperative pain and resulted in earlier recovery, when compared to conventional saphenectomy.
