Endovascular Treatment of Pulmonary and Cerebral Arteriovenous Malformations in Patients Affected by Hereditary Haemorrhagic Teleangiectasia

Abstract: Hereditary Haemorrhagic Teleangiectasia (HHT) is a vascular disorder of angiogenesis transmitted in an autosomal dominant pattern, characterised by heterogeneity in clinical manifestations. One of the most important organ involved is lung, including pulmonary arteriovenous malformations (PAVM). PAVM occur in 20 to 30% of the HHT population and recently are considered a marker of disease. PAVM are direct artery-to-vein connections with low pressure and without an interveining capillary bed. PAVM are classified … Show more

“…In addition, intravoxel incoherent motion and diffusion magnetic resonance imaging, as well as fluorodeoxyglucose positron emission tomography, have great potential to distinguish between non‐invasive and invasive tumors . High‐resolution superselective angiography has been widely applied in the diagnosis and treatment of vascular diseases, such as pulmonary arteriovenous malformation, gastrointestinal hemorrhage to perform both superselective angiography of the feeding artery, and embolization itself . The practicability of these techniques for a giant thoracic neoplasm is worth studying.…”

“…The medical literature provides little information on manifestations of HHT in children [1]. Occasionally, HHT can also be involved with cerebral AVM and pulmonary or gastrointestinal vasculature.…”

“…However, ionizing radiation is a potential drawback to the routine use of CT for pulmonary AVM screening and followup, particularly in young subjects or women of child-bearing age, and particularly if patients require regular follow-up over an extended period. For the same reasons, and because of the high cost, time requirements, and inherent complications, conventional pulmonary angiography is inappropriate as a screening tool and is today usually used solely for preinterventional localization during embolization therapy [37].…”

“…Of the various approaches to the treatment of pulmonary AVMs, coil embolization is widely considered the safest and most effective technique (37,(46)(47)(48)(58)(59)(60)(61). However, successful embolization depends on satisfactory visualization of the feeding vessels of the pulmonary AVM to adequately position the embolization catheter.…”
