Endovascular treatment modalities for basilar artery fenestration aneurysms: experience of two centers and literature review

Abstract: IntroductionBasilar artery fenestration (BAF) is a rare vascular variation and its incidence is reported as 0.6-2.33% depending on the diagnostic modality chosen [1][2][3]. Fenestrations on this region increase tendency to aneurysm generation [2]. Complex geometric structure of the fenestration, proximity to lower cranial nerves, presence of sort of vital arterial structures, and deep and challenging nature of this region for surgery increase mortality and morbidity of surgical treatment in these aneurysms [4]… Show more

“…7 No specific angiographic FU and AOR were available. 4,5,8 The immediate AOR in our study is comparable to those described in the literature with complete and successful AOR of 69% (v. 66–91%) and 83% (v. 75–100%), respectively. 48 However, a comparison between the studies is difficult as treatment strategies have changed over time and new equipment for endovascular therapy of intracranial aneurysms has come to the market including different types of microcatheters, stents and flow diverting devices.…”

“…flow diverters were used. 8 In the present study, aneurysms also were treated with intra-saccular flow disruption (WEB), which was recently described as feasible treatment option for this type of lesion. 11…”

“…Aneurysms are classified based on their size as small (diameter less than 11 mm), large (diameter of 11-25 mm), or giant (diameter greater than 25 mm) [ 7 ]. The treatment options for aneurysms include standard coiling, balloon-assisted therapies, stent-assisted therapies, and flow-diverter systems [ 8 ]. Vajpeyee et al in 2013 used a novel double microcatheter-assisted technique for coiling a BAF aneurysm [ 9 ].…”

“…One patient had a permanent neurological deficit, and another patient died following surgical treatment [ 12 ]. In 2021, a comprehensive review of literature by Korkmaz et al reaffirmed the notion that ET is superior to a microsurgical approach in terms of higher clinical success rate and lower complication rate [ 8 ].…”
