Endovascular Renal Artery Stent Retriever Embolectomy in a Young Patient With Cardiac Myxoma: Case Report and Review of the Literature

Abstract: We describe 2-month renal function outcomes and the considerations in applying a neuroendovascular mechanical thrombectomy device within the renal artery. This was a radiographically successful technique and her renal function appears to be improving at the 2-month follow-up, although this is complicated by other renal insults and support.

“…A retrospective review of 436 acute ischemic stroke patients who underwent mechanical thrombectomy using ADAPT revealed a reasonable treatment for the patients with acute BAO. 13 15) In the previous case reports about the mechanical thrombectomy for cardiac tumor and stent retriever, 8 , 9) ADAPT was successfully performed. 10 , 11) Regarding the present case, ADAPT technique would be recommended when the patient acute cerebral artery occlusion associated with a radiological finding of lung tumor invading pulmonary vein because the thrombi might be too hard to be grasped by stent retriever.…”

“…The microcatheters usually have .021 to .027-inch inner diameters. 6 Complications of SR use include arterial perforation (.9-4.9%), intracerebral hemorrhage (3.6-9.3%), subarachnoid hemorrhage (.6-4.9%), arterial dissection (.6-3.9%), distal embolization (1-8.6%), vasospasm (3.9-23%), clot fragmentation (rare), and stenosis. 10 Multiple SR designs exist in the market and several experiments have tested the efficacy of thrombus removal based on different device components and techniques.…”

“…1 Specifically, interventional radiologists have previously implemented direct thrombolysis, aspiration, stenting, angioplasty, snaring, and rheolytic mechanical thrombectomy as a form of endovascular treatment. 6 Surgery is less common due to its higher morbidity and mortality but should be considered in patients with peritonitis or with evidence of infarcted bowel as it allows for direct bowel exploration and potential resection. 5 Innovative means of removing intra-abdominal arterial occlusions have included the off-label use of stent retrievers (SRs), endovascular tools traditionally used for management of intracranial vascular thrombosis in patients with acute strokes.…”

“…22 Stenting after thrombectomy is less common. 31 It is advisable to choose treatment tactics based on the degree of thrombosis. In case of incomplete thrombosis, conservative anticoagulant therapy demonstrates good results (renal function decrease after 2.5 years by 9%), complete thrombosis requires endovascular thrombectomy (renal function decrease after a year by 27%).…”
