Endoscopy in inflammatory bowel disease: from guidelines to real life

Abstract: Endoscopy has a central role in the management of inflammatory bowel disease (IBD), providing crucial data for diagnostic and therapeutic decisions, treating disease-related complications, and assisting in the early detection of dysplasia and prevention of colorectal cancer in the setting of IBD. Treatment targets have significantly shifted in IBD, focusing on achieving mucosal healing, a more meaningful endpoint than clinical remission. With the emergence of novel therapies, we aim to alter the course of the … Show more

“…Up to the moment of this review, limited information is available regarding the in vivo effects of cannabinoids on intestinal inflammatory conditions in humans. The outcome predictors in IBD trials usually include DAI, C-reactive protein (CRP), rapid fecal calprotectin (FC), the Mayo score and the Crohn's Disease Activity Index (CDAI), and the correct interpretation of the results relies on quality preparation for colonoscopy [81][82][83].…”

“…However, in follow-up studies, a higher percentage/risk of relapse has been observed in patients who had reached a MES 1 index compared to the group with a MES 0 index (36.6% vs 9.4% with P < 0.001)[ 13 ]. On the other hand, an endoscopic response has been defined as a decrease in ≥ 1 degrees of the MES or a decrease in ≥ 2 points of the UCEIS[ 14 , 15 ].…”

“…Endoscopy plays an important role in the diagnosis and treatment of patients with inflammatory bowel disease (IBD) because it can assess the diagnosis and activity of the disease. 36 The current treatment strategy for ulcerative colitis (UC) patients aims to achieve mucosal healing, and the degree of mucosal healing is assessed and graded by a number of endoscopic indicators, including Mayo endoscopic score (MES) and ulcerative colitis endoscopic index of severity (UCEIS). However, many studies have shown that not only achieving mucosal healing, but also further achieving the goal of histological remission may lead to better treatment and prognosis.…”
