Endoscopic ultrasound-guided gastroenterostomy versus duodenal stenting for malignant gastric outlet obstruction: an international, multicenter, propensity score-matched comparison

Abstract: Background Endoscopic duodenal stenting is the current standard treatment for malignant gastric outlet obstruction (GOO) in patients with limited life expectancy. However, duodenal stenting is prone to stent dysfunction. Endoscopic ultrasound-guided gastroenterostomy (EUS-GE) is a novel technique with potentially superior stent patency. We compared clinical success, safety, and stent dysfunction of EUS-GE and duodenal stenting in patients with malignant GOO using propensity score matching. Methods Th… Show more

“…Although surgical gastrojejunostomy has shown better long‐term results, ES is still widely used for malignant GOO, 20 despite providing suboptimal relief from symptoms and a higher risk of recurrence. 8 EUS‐GE is increasingly being used as an alternative to both ES and surgical bypass, as it avoids the invasiveness of surgery and the risk of primary failure, recurrence, and need for re‐interventions following ES. 8 , 16 …”

“…In this issue of Endoscopy , the nicely conducted study by van Wanrooij et al compared the efficacy, safety, and stent dysfunction rates of EUS-GE vs. duodenal stenting in patients with malignant GOO using propensity score matching 8 . The study was conducted between 2015 and 2021 across three European centers with expertise in EUS-guided interventions.…”
