Endoscopic Transaqueductal Placement of a Single-Catheter Cyst-Ventriculoperitoneal Shunt in a Neonate With Dandy-Walker Malformation-Associated Hydrocephalus -Case Report-

Abstract: A neonate with hydrocephalus associated with Dandy-Walker malformation was successfully treated with an endoscopic placement of a transaqueductal ventricular single catheter. The modified catheter was provided with additional fenestration on its proximal side to allow simultaneous drainage from both the supra-and infratentorial compartments. This technique is well known for isolated fourth ventricles, but has not been applied to hydrocephalus associated with Dandy-Walker malformation. The cyst-ventriculoperito… Show more

“…Though transaqueductal intracystic inspections with flexible endoscopes and aqueductal stent placement were performed quite frequently, intracystic catheter placement via the aqueduct was only used by Morigaki et al [25], who used a ventricular catheter with an Innervision system and an outer diameter of 2.5 mm. Our approach can be performed through a posterior fossa burr hole, aqueductal flow can be inspected and a catheter ascending through the aqueduct can be gently advanced to the 3rd or lateral ventricles.…”

“…19 Effective endoscopic third ventriculostomy caused a small lessening in ventricle size and changing degrees of decrease in size of cyst. 20 It is also distinguished that failure rates of third ventriculostomy are stated to be chiefly great in the first year of age 22,23.…”
