Endoscopic transantral repair of orbital floor fractures

Abstract: The described technique of endoscopic repair of orbital floor fractures represents a precise method of fracture repair that results in excellent outcomes with minimal morbidity in the majority of patients. It allows for immediate fracture repair without the need to wait for periorbital edema to settle. It also allows for clear visualization of the entire fracture for precise graft placement.

“…It is emphasised that the region of the infraorbital canal and groove is crucial in surgical reconstructions [1]. Manipulations in this region during such procedures as rhinoplasty, tumour surgery of the maxilla and molar fractures and Le Fort I type osteotomies may result in an iatrogenic injury to the infraorbital nerve and vessels [9,14,18,22].…”

“…Nine studies 1,2,5,[8][9][10][11][12][13] met the criterion for inclusion in the systematic review; 2 were comparative studies comparing the endoscopic with the external approach and 7 were case series ( Table 1). All studies were retrospective except for 1 case series.…”

“…Both endonasal and transantral (through a gingivobuccal incision and Caldwell-Luc antrostomy) approaches to the maxillary sinus have been described. 1,2 Although this is not a novel concept, 3 advances in endoscopic technology (improvement in illumination and image transmission) have allowed for excellent visibility and a higher-definition picture. Following assessment of the defect from below, herniating orbital fat and muscle can be gently reduced with an instrument.…”

“…1,2 Surgical repair of these fractures is indicated for persistent diplopia, soft tissue or extraocular muscle entrapment, enopthalmos, oculocardiac reflexes, and large defects, although the size threshold for intervention remains contested. 4 Periorbital approaches offer visualization of the defect and orbital contents, providing excellent access for repair. 4 Periorbital approaches offer visualization of the defect and orbital contents, providing excellent access for repair.…”
