Endoscopic Thyroidectomy With the da Vinci Robot System Using the Bilateral Axillary Breast Approach (BABA) Technique

Abstract: Robotic endoscopic thyroidectomy using the BABA technique is a feasible procedure and can be performed safely. It provides an excellent operative field view enabling easy identification of vital structures. It also gives the desired cosmetic results and minimal postoperative pain similar to conventional endoscopic thyroid surgery using the BABA technique.

“…The currently published series have a temporary RLN injury rate between 3 and 20 %, with permanent injury rates ranging between 0.7 and 1 % [25 • , 34,20,44,45,43,29,30,23]. The permanent rates are similar to rates for open surgery, where it is generally agreed that rates of less than 2 % are acceptable.…”

“…Bilateral Axillary Breast Approach Lee et al [30] described a bilateral axillary and breast approach (BABA) of endoscopic thyroid surgery with the da Vinci robotic platform in 2009. Patients were selected with low-risk thyroid cancer, nodules with benign cytology up to 8 cm, and Grave's disease.…”

“…The use of robot techniques is new in the field of thyroid surgery. The few studies that have examined robotic thyroidectomy have involved only a small number of patients in a single center [9,[11][12][13][14][15]. Although other reports have demonstrated the safety and feasibility of robotic thyroidectomy, the present work is the first multicenter study to investigate this approach specifically for thyroid carcinoma treatment.…”

“…The few published studies in this field are relatively small [11][12][13][14]. Little is known regarding the efficacy and safety of this technique, especially during the time of its introduction into centers and during the surgeon ''learning'' phase.…”

“…These include thyroidectomy through chest (6), breast (7), axillary (8,9), bilateral axillo-breast (10), and retroauricular (11) incisions. Also, robotic facelift thyroidectomy, a procedure that combines thyroidectomy with retroauricular facelift to achieve superior cosmesis, has been described (12).…”
