Endoscopic Surgery of Inverted Papillomas under Image Guidance—A Prospective Study of 42 Consecutive Cases at a Danish University Clinic

Abstract: A treatment based on endoscopic resection with image guidance appears to offer a safe treatment modality of inverted papilloma with insignificant morbidity. Predominantly cases with nonmedial involvement of the maxillary sinus still require a supplement with the Caldwell Luc procedure. Although all the recurrences were found within 9 months postoperatively, later recurrences cannot be excluded. Long-term follow-up is recommended.

“…The aim is thorough excision combined with minimal treatment morbidity. In unselected longitudinal series with inverted papilloma the recurrence rate is found to be 0-25% with a minimal treatment morbidity and, interestingly, with no reported cases of malignant transformation [15][16][17]. The same surgical procedures have recently been introduced in the clinic in the treatment of selected malignant sinonasal lesions including inverted papilloma with SCC.…”

“…Parallel with this, the risk of malignant transformation (metachronous lesion) should be reduced when complete excision is achieved. Traditional surgical procedures comprising external approaches like lateral rhinotomy and midfacial degloving have more or less been substituted by minimally invasive procedures using combinations of endoscopy, image guidance and powered instrumentation [15][16][17]. The aim is thorough excision combined with minimal treatment morbidity.…”

“…The piecemeal approach, inherent with endoscopic surgery, could be acceptable because what is most important is to identify and widely remove the site of attachment (Han et al, 2001;Kraft et al, 2003). Recently, the use of navigation systems for endoscopic removal of IP has been reported (Von Buchwald and Larsen, 2005) but recurrences were still reported.…”

“…Nowadays, endoscopic sinus surgery is the best treatment option for initial and even more advanced papillomas. Contraindications of endoscopic surgery include the presence of squamous cell carcinoma, intradural or orbital extension [18][19][20][21] . In our study, most cases, staged as Stage III Krouse, were treated by endoscopic surgery by medial maxillectomy and sometimes followed by anterior and posterior ethmoidectomy.…”
