Endoscopic submucosal dissection versus surgical resection for early gastric cancer: a retrospective multicenter study on immediate and long-term outcome over 5 years

Abstract: ESD might be an acceptable and effective treatment for EGC considering overall survival rates with fewer early complication rates and shorter duration of hospital stay compared to surgical resection. However, intensive and persistent endoscopic surveillance should be performed after ESD for early detection of metachronous lesions.

“…Large series from East Asia show high en bloc resection (86–97%) and curative resection (R0) rates (88–93%) for EGC treated by ESD . Local recurrence rate after gastric ESD is approximately 1–2% per annum, with 5‐year overall survival of 96–100% and 5‐year disease‐free survival of 97–100% . Two meta‐analyses evaluating ESD versus EMR for treatment of EGC found higher rates of en bloc (92% vs 52%, respectively) and R0 (82–92% vs 42–43%, respectively) resection and less local recurrence (0.8% vs 5.0–6.4%, respectively) .…”

“…Delayed bleeding rate was 6%, and perforation rate was 1% . In comparison, large multicenter studies of ESD for EGC at major Eastern institutions report similar en bloc resection rates of 92.7–96.1% and R0 resection rates of 82.6–94.5%, with bleeding rates of 0.6–9.9% and perforation rates of 3.6–4.7% …”
