Endoscopic submucosal dissection/endoscopic mucosal resection guidelines for esophageal cancer

Abstract: The Japan Gastroenterological Endoscopy Society has developed endoscopic submucosal dissection/endoscopic mucosal resection guidelines. These guidelines present recommendations in response to 18 clinical questions concerning the preoperative diagnosis, indications, resection methods, curability assessment, and surveillance of patients undergoing endoscopic resection for esophageal cancers based on a systematic review of the scientific literature.

“…However, indications for ESD have been extended with the recent development of treatments for stenosis, including endoscopic balloon dilatation and local injection or oral administration of steroids. 16 Based on these findings, ESD is considered for the treatment of patients with superficial esophageal neoplasia. Esophageal ESD was adopted as a treatment covered by the medical insurance in Japan in 2008 and is performed in general hospitals and advanced facilities.…”

“…Acetic acid staining can also be helpful to evaluate the extent of Barrett´s cancer underneath the squamous epithelium [20]. In contrast to endoscopic image analysis (in particular with magnifying endoscopy), endoscopic ultrasound is less accurate in the prediction of infiltration depth [21]. However, endoscopic ultrasound is the most reliable method to detect possible lymph node metastasis and thus can be helpful in selected cases [22].…”

“…Therefore, EMR is the preferred resection technique in Western countries [30] (Figures 2 a-d). In contrast, endoscopic submucosal dissection (ESD) is the recommended method in the Japanese guidelines [21]. In fact, ESD is more appropriate for larger lesions, in particular Paris Type 0-Is, and is recommended for suspicious lesions for which en bloc resection is not possible using endoscopic mucosal resection [8,31].…”

“…7 Consequently, a guideline for endoscopic resection in esophageal cancer (which systematically determined the effectiveness of PPI following esophageal ESD) weakly recommend that PPI should not be administered for esophageal ESD, except in patients with GERD symptoms or reflux esophagitis. 8 However, further research is required because this recommendation is based on only one study. Therefore, the objective of this study was to evaluate the efficacy of PPI in terms of ulcer healing following ESD for superficial esophageal squamous cell carcinoma, using a propensity score analytic approach.…”
