Endoscopic sphincteroplasty: a novel and safe alternative to papillotomy in the management of bile duct stones.

Abstract: Removal of bile duct stones during endoscopic retrograde cholangiopancreatography (ERCP) usually includes papillotomy. Papillotomy is associated with occasional complications and in addition, the longterm sequelae of papillotomy in young patients having laparoscopic cholecystectomy remain unclear. As an alternative to papillotomy, this study prospectively evaluated the efficacy and safety of endoscopic balloon sphincteroplasty to facilitate bile duct clearance. Of 32 patients with bile duct stones (diameter 3-… Show more

“…Ersoz et al [13] were the first investigators to use sequential EST and EPBD using large-diameter balloons for sphincteroplasty. All of the studies on EPBD alone had used balloons of 8 and 10 mm in diameter, which obviously limited the extraction of large stones [18][19][20][21]. Ersoz et al chose 58 patients who had failed EST and stone extraction, and dilated the sphincter with 10-20-mm balloons.…”

“…Endoscopic removal of small MBD stones through the intact papilla after or without pneumatic dilatation of the sphincter is being increasingly performed. 20,21 Whether there will be long-term consequences to the sphincter from this approach remains to be seen. Most complications related to LC, such as residual stones and biliary leaks, can be treated effectively by endoscopy.…”

“…Endoscopic sphincterotomy (EST) is accepted as the standard management for stone removal from the bile duct, but it is associated with serious complications such as hemorrhage, pancreatitis, perforation, and recurrent infection of the bile duct, which cause permanent functional loss of the sphincter of Oddi [1][2][3][4] . Endoscopic papillary balloon dilation (EBD) was introduced by Staritz et al [5] and has been accepted widely as an alternative to EST [6][7][8][9][10] . It has similar outcomes for common bile duct stone removal compared to EST, and has the advantages over EST of preserving papillary sphincter function and causing minimal complications such as hemorrhage and perforation [11][12][13][14][15][16][17][18][19] .…”
