Endoscopic Snare Papillectomy in Patients with Familial Adenomatous Polyposis and Ampullary Adenoma

Abstract: The optimal treatment of adenomas of the papilla of Vater has still not been definitively established, and the endoscopic excision of such lesions has received little attention in the literature. We report here the cases of two patients with familial adenomatous polyposis, in whom ampullary adenomas measuring 8 and 20 mm, respectively, were treated using one-piece snare excision of the lesion together with the papilla (snare papillectomy), followed by temporary biliopancreatic drainage. Procedure-related compl… Show more

“…Ampullary adenoma represents the precursor lesion to adenocarcinoma, and a coexistent adenoma can be identified frequently in patients with ampullary adenocarcinoma (1, 2). Because of their premalignant nature, ampullary adenomas are often resected either by polypectomy or, in the case of larger lesions, by pancreaticoduodenectomy (1,4,5). An association between adenomas of the ampullary region and pancreatic intraductal neoplasia has not been documented previously.…”

“…Diagnostic papillectomy (or papillotomy in selected cases) including deep biopsy is recommended if there are an infi ltrating tumor growth revealed by EUS and negative fi ndings in the initial histologic investigation. Unlike laser or thermal ablation, papillectomy allows complete histologic investigation of the pathologic tissue (Bertoni et al 1997). However, decision-making for a minimally invasive endoscopic approach is infl uenced by the spectrum and risk of possible complications such as perforation or bleeding.…”
