Endoscopic Retrograde Cholangiopancreatography and Needle-Knife Sphincterotomy in Patients with Billroth II Gastrectomy: A Comparative Study of the Forward-viewing Endoscope and the Side-viewing Duodenoscope

Abstract: For ERCP and EST in patients with Billroth II gastrectomy, the forward-viewing endoscope is as effective as the side-viewing duodenoscope, but it may be the safer of the two instruments. The forward-viewing endoscope can be recommended for ERCP and EST in patients with a Billroth II gastrectomy.

“…Th e fi rst use of the endoscopic approach in patients with Billroth II anatomy was reported by Katon et al ( 6 ) in 1975. Subsequent attempts have been made to perform ERC using standard side-viewing duodenoscopes, forward-viewing endoscopes and colonoscopes, with success rates of 50-92% ( 3,(7)(8)(9)(10)(11)(12). Th e fi rst use of ERC in patients who had undergone Rouxen-Y (R-Y) reconstruction was reported by Gostout and Bender in 1988 ( 13 ); subsequent attempts to perform ERC with duodenoscopes, pediatric-or adult-use colonoscopes, enteroscopes, ( 12,(14)(15)(16) and oblique-viewing endoscopes with an overtube ( 17 ) have achieved success rates of 33-67%.…”

“…176,177 However, smaller case series report perforation rates as high as 18%, which may reflect closer to the experience of community ERCP endoscopists. 178 Post-Whipple (pancreaticoduodenectomy) anatomy does not appear to significantly increase the risk of perforation during ERCP. 179 It is important to consider that various risk factors may predispose to each type of perforation.…”

“…However, the perforation rate was significantly lower with the forward viewing endoscopes. 21 In order to improve orientation for cannulation in Biliroth II, several ERCP accessories such as a rotatable sphincterotome, a sigmoid shaped sphincterotome or a straight catheter with groomed distal tip have shown good efficacy (~90%) with low adverse event rates (< 5%). 22,23 Another popular strategy is the use biliary stent as a guide to perform needle knife sphincterotomy with a reported success rate of 83% and early complication rate of 39% (bleeding 16%, pancreatitis 8%, perforation 2%) in a small cohort of 18 patients.…”
