Endoscopic resection of a giant fibrovascular polyp of the oesophagus with the assistance of ultrasonic shears

Abstract: SUMMARYGiant fibrovascular polyps of the oesophagus are rare benign tumours originating from the upper oesophagus. A 58-year-old woman presented with a 6-week history of a sore throat, odynophagia and progressive dysphagia, managing only a soft diet. CT of the neck and thorax, and barium swallow, both demonstrated a giant fibrovascular polyp measuring approximately 7 cm in length arising from the proximal oesophagus. The patient underwent endoscopic resection of the polyp with the assistance of ultrasonic shea… Show more

“…Small polyps with a clearly identifiable stalk may be accessible by rigid or flexible endoscopy and excised with either by snare, ligature, endo-GIA stapler, Nd:YAG laser, or ultrasonic shears [3]. Larger lesions longer than 10 cm may be difficult to retrieve endoscopically and are better treated with surgical resection.…”

“…2 Polyp resection is recommended in all patients for controlling symptoms and eliminating the risk of asphyxiation. 3 Traditional excision commonly involves transcervical vertical esophagotomy. Endoscopic resection is technically feasible and avoids transcervical vertical esophagotomy-related morbidity.…”

“…Endoscopic resection is technically feasible and avoids transcervical vertical esophagotomy-related morbidity. 3 Recently, endoscopic resection with ultrasonic shears, a needle-type knife, and an electrosurgical snare has been performed. [3][4][5] The Scissortype knife facilitates grasping the pedicle and enables simultaneous excision and coagulation while maintaining a stable view by use of only a single device.…”
