Endoscopic papillectomy of minor papillar adenoma associated with pancreas divisum

Abstract: Tumors of the minor papilla of the duodenum are quite rare. We successfully and safely treated an 18-mm adenoma of the minor papilla associated with pancreas divisum using endoscopic papillectomy. A 64-year-old man was admitted to our hospital for treatment of an asymptomatic mass in the minor papilla detected by upper gastrointestinal endoscopy. Endscopic analysis showed an 18-mm, whitish, sessile mass, located in the duodenum proximal to a normal-appearing major papilla. Endoscopic retrograde pancreatography… Show more

“…Using no touch isolation, the tumor was not exposed to the operative field as in a transduodenal local resection, and the whole layer of the duodenum could be resected. In addition, several cases of endoscopic resection for neoplasms of minor duodenal papilla such as adenomas [35,36], carcinoid tumors [37], or gangliocytic paraganglioma [38,39] have been previously reported. Early and exact preoperative diagnosis of duodenal neoplasms makes it possible to select an appropriate surgical strategy, which also maintains curability.…”

“…Most MiDP tumors are reported to be adenomas (8)(9)(10)(11)(12), neuroendocrine tumors (13,14), or rarely, ganglyocytic paragangliomas (15,16). MiDP adenocarcinoma is extremely rare.…”

“…Tumors of the minor papilla are uncommon and, to date, are only described in case reports. 1 – 7 In contrast, tumors of the major papilla account for approximately 5% of gastrointestinal (GI) neoplasms. 8 Duodenal adenomas may occur as sporadic lesions or in patients with familial adenomatous polyposis (FAP).…”
